H&W ICB is responsible for planning and allocating resources to meet the four core purposes of the Integrated Care System, namely:

  • to improve outcomes in population health;
- tackle inequalities in outcomes, experience, access;
- enhance productivity, efficiency, value for money;
- support broader social and economic development.

Within this context, as NHS H&W develops as a statutory organisation, a significant challenge will be to determine how it effectively supports and enables place-based and local partnerships to take necessary action that will improve population health outcomes and reduce inequalities (i.e. the unfair and avoidable differences in health across the population, and within society).

General Practice has a key role to play as equal partners within the ICS and the postholder will play a key role in development and implementation of a 5 year General Practice Strategy, Fuller Stocktake implementation, general practice workforce and sustainability.


Integrated Care Systems (ICSs) are partnerships of health and care organisations that together, plan and deliver joined up services to improve the health and wellbeing of people who live and work in their area.

Their purpose is to improve outcomes in population health & healthcare; tackle inequalities, enhance productivity and value for money and support broader social & economic development in their area.

This will be delivered in neighbourhood, place, combinations of places and system.


The Head of Primary Care Transformation will lead, support and influence a wide range of transformation programmes by working in collaboration with all relevant stakeholders, and especially General Practice.

The role will support the delivery of the provider partnership work programmes that will underpin the strategic transformational change required.

The post holder will drive, deliver and influence transformational change which entails leading on the development of strategic and tactical approaches, using effective partnerships to ensure these become operational.

It requires new and innovative approaches to commissioning that meet the health and social care needs of the population and deliver against national and local performance targets.

With responsibility for delivering the transformation of services, the post holder will provide assurance, support and advice to the Director of Primary Care Transformation and Place-based PCN Directors of Strategy and Partnership.

The post holder will provide expert advice and leadership on the Primary Care Workforce Development programmes, commissioning process, including identifying opportunities for collaborative working.

The post holder will participate in the director on call rota.

  • General Practice Strategy development & implementation
  • working with the Director of Primary Care to develop and implement a Primary Care strategy, aligned with the Fuller Stocktake, national policy and contract reform and the NHS Long Term Plan's requirements for Primary Care.
  • Fuller Stocktake implementation supporting implementation of the Fuller Framework putting in place action plans, milestones and outcomes.
  • General Practice Sustainability and Transformation
  • Workforce Development programmes, building workforce capacity, resilience and skills including workforce planning, recruitment and retention.
