About the role


To work within a dynamic team providing evidence-based, specialised home oxygen assessment, interpretation and safe oxygen therapy prescription for community patients in their own homes and clinical environments.

The post holder will be responsible for building and maintaining relationships with key healthcare professionals whilst working to a high degree of accuracy and adhering to contractual requirements.


What you will be doing

  • Work as an autonomous practitioner in partnership with other health/social care professionals, using specialised skills and knowledge to assess, monitor and review the physical, social and psychological needs of the patients with respiratory diagnosis.
  • Develop care plans and instigate therapeutic treatments based on comprehensive patientfocused assessments and best available evidence (national guidelines, latest research and expert opinion) to improve health outcomes.
  • Assessment of patients for long term and or ambulatory oxygen therapy in accordance with National Guidelines.
  • Empower patients to actively manage their own conditions, improving their quality of life and encouraging behaviour change strategies e

g:
smoking cessation, pulmonary rehabilitation, home exercise regimes, improved concordance with oxygen, medications and symptom management.

  • To reduce avoidable and inappropriate hospital admissions/readmissions, by assessing patients in a timely manner and working with other health and social care teams, to support the care of the patient in their own home wherever possible.
  • To undertake community oxygen assessments, including the use of capillary blood gases, their interpretation and appropriate prescription of oxygen delivery systems. To communicate the results of the assessments to the GP's hospital respiratory services and other clinical staff involved with the patient care.
  • To work as a nonmedical prescriber, linking where appropriate to other professionals involved in medicines management such as GP's and Pharmacists.
  • To support the clinical lead in delivery of the home oxygen assessment service.
  • Manage and prioritise the ongoing care of patients to ensure the efficient and optimal use of resources.
  • To provide advice, teaching and instruction to patients, relatives, carers and other professionals, to promote health and wellbeing, and to ensure a consistent, safe approach to patient care.
  • To ensure personal knowledge, skills and competency are updated and maintained through appropriate CPD, training and development opportunities to enable effective care to be delivered in line with current evidence and practice.
  • In conjunction with other care professionals, to develop and regularly review patient information and documentation using a variety of mediums that considers cultural diversity and the varying communication needs of patients and users.
  • Ensure a safe environment for the treatment of clients and protection of staff through risk assessment and by ensuring the effective use of equipment and materials in compliance with health and safety and other relevant policies.
  • Complete enhanced risk assessments and safety interventions to ensure the safe delivery of oxygen therapy.
  • Use electronic medical recording systems to ensure that clinical records are kept that support clinical needs.
  • Enable audit and evaluation of the service whilst safeguarding patients' rights and confidentiality
  • Support Baywater Healthcare home oxygen service NHS stakeholders with safe oxygen prescribing one day per week.
  • Offer support as needed to Baywater Healthcare Pulmonary Rehabilitation Services.

About You
Qualified and NMC registered nurse or similar qualification and experience

  • Experience in respiratory medicine
  • Specialist clinical assessment skills
  • Ability to drive and access to a car for business purposes
  • Mentorship/Teaching skills
  • Computer literacy and confidence to work with all Microsoft office packages, eg, Word, PowerPoint, Excel
  • Training and educational skills
  • Demonstrable track record of achievement in your field
  • Evidence of continual professional development and training to meet NMC revalidation requirements
