    Job summary

    Thepost holder will be a pharmacist, who acts within their professionalboundaries, supporting and working alongside a team of clinicians in generalpractice. In this role they will be supported by a Senior Clinical Pharmacist who will assist the surgerys clinical lead todevelop, manage and mentor them. Theywill be employed by, and based at, Beccles Medical Centre. The successfulapplicant will join a large, established team of Prescribing Pharmacists andPharmacy Technicians.

    Thisrole would be suitable for a newly-qualified Pharmacist, who is looking todevelop their knowledge and skills in primary care. Since this role isavailable under the NHSE ARRS scheme, the post-holder would qualify for thePrimary Care Pharmacy Education Pathway (PCPEP), an 18 month education anddevelopment programme provided by Health Education England.     Thepost holder will work as part of a multi-disciplinary team in a patient-facingrole. The post holder will be involved with chronic disease management and willundertake clinical medication reviews to proactively manage patients withcomplex polypharmacy. The post holder will provide primary support to general practice staffwith regards to prescription and medication queries.

    Main duties of the job

    See (whereappropriate) patients with single or multiple medical problems where medicineoptimisation is required ( COPD, asthma).Review theon-going need for each medicine, a review of monitoring needs and anopportunity to support patients with their medicines taking ensuring they getthe best use of their medicines (medicines optimisation). Make appropriaterecommendations to Senior Pharmacists or GPs for medicine improvement.

    Undertake clinical medication reviews with patients and producerecommendations for nurses and/or GP on prescribing and monitoring.

    Managing caseload of patients with common/minor/self-limiting ailmentswhile working within a scope of practice and limits of competence.

    Provide patient facing clinics for those with questions, queries andconcerns about their medicines in the practice.

    Answer relevant medicine related enquiries from GPs, other practicestaff, other healthcare teams ( community pharmacy) and patients withqueries about medicines.

    Review the use of medicines most commonly associated with unplannedhospital admissions and readmissions through audit and individual patientreviews.

    Knowledge, Skills and ExperienceRequired

    Completion of an undergraduate degree inpharmacy and registration with the General Pharmaceutical Council.

    Have experience and an awareness ofcommon acute and long-term conditions that are likely to be seen in generalpractice.

    May hold or be working towards anindependent prescribing qualification, or other post-graduate qualification.

    Recognises priorities whenproblem-solving and identifies deviations from normal pattern and is able torefer to seniors or GPs when appropriate.

    Able to follow legal, ethical,professional and organisational policies/procedures and codes of conduct.

    Involves patients in decisions aboutprescribed medicines and supporting adherence as per NICE guidelines.

    Job Description Agreement

    Qualifications

    Essential

  • Masters degree in pharmacy (MPharm) (or equivalent)
  • Mandatory registration with General Pharmaceutical Council
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
  • An appreciation of the nature of GPs and general practices
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/ agreed course of action where there may be significant barriers
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients)
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
