    ALLDAY EVERY DAY WE MAKE LIFE BETTER

  • Observe eachresident on a continuing basis alert to changes in mental andphysical wellbeing; notifies the Supervisor of pertinentobservations and discusses the need to notify physician; andreceive Resident Status Report from offgoing shift and giveoncoming shift a detailed report of each resident s status andevents of the shift.
  • Accurately receivetranscribe and carry out physician sorders.
  • Administer medications and treatmentsas ordered by the physician and make proper documentation via acomputerized system of MARs/TARs and electronic documentation.Order and maintain all pharmaceuticals for the units.
  • Supervise LNAs in accordance with licensingrequirements under the New Hampshire Board of Nursing. Assign andmeasure nursing staff resident care consistent with abilities jobdescription and scope of practice and supervise the care given; andassist directcare staff with nursing care as necessary and as timeallows.
  • Accompany physician on resident roundson their assigned unit(s) to address residents medical needs and tofacilitate communication during 60day review.
  • Administer the provision of oxygen inaccordance with facility guidelines and procedures. Administergastrostomy/jejunostomy feedings in accordance with policy andprocedure. Perform wound care and have knowledge of woundvacs.
  • Perform regular duties on the unit suchas checking appointments for the day; checking PhysicalOccupational and Speech Therapy appointments; checking activityboard; reminding staff of ambulation list weights blood pressuresand to document nutritional charts. Monitor LNA documentation andupdate LNA clipboard; oversight of infection control practices onthe unit; ensure that unit is clean and hazardfree. Following thechain of command report any problems to the Nurse Manager/NursingSupervisor. Encourage LNA observations and include them whendocumenting and reporting.
  • Admit discharge andtransfer residents completing documentation and notifications asnecessary.
  • Complete 24hour report for his/herassigned unit(s); check new orders; complete list of residents onantibiotics precautions etc.; maintain consistent communicationwith the Nurse Manager/ Nursing Supervisor regarding residentissues or concerns with staff; differentiating between acute andnonemergency problems; and attend all mandatory staff/educationalmeetings and nonmandatory meetings asassigned.
  • Communicate with residents familiesto resolve resident concerns and problems to notify family membersof new orders and to obtain consent when necessary.
  • Receive incoming calls and determineappropriate response required.
  • Assumeresponsibility for the management of assigned unit(s) duringhis/her shift under the direction of the Nursing Supervisor (NS) orDirector of Nursing Services (DON).
  • FollowState and Federal Regulations regarding the storage usage anddocumentation of controlled medications including counting at thechange of shift; notifies appropriate Supervisor if discrepanciesare present.
  • Assist with the orientation ofnew nursing employees on their assigned unit(s). Communicate withDON Nurse Manager or SDC regarding continuing education needs ofthe nursing staff and assist in the delivery of staff education andtraining as needed. Encourage and assign staff to attend in serviceeducation meetings. Evaluate assigned nursing staff membersaccording to facility policy and procedure. Initiate or effectivelyrecommend disciplinary action asnecessary.
