    Clinical Nurse Specialist for Physical Health and Wellbeing
    Band 7

    Main area Mental Health - Child and Adolescent Grade Band 7 Contract Permanent Hours Full time - 37.Clare Lodge is a secure children's home located on the outskirts of Peterborough.

    We work within a trauma informed model of care with a group of children females only) who have been placed by the courts in order to secure their welfare and improve their overall wellbeing.

    The unit is run by the local authority and we operate as a in reach health team commissioned by NHSE and located onsite.

    Our work has developed over the past 2 years and we now have a large multi- disciplinary team consisting of Psychiatry, Psychology, Nursing, AHP's and admin support.

    We are a supportive, friendly, and committed team working within a wider team of local authority and education staff and we are looking for a like minded CNS to take a lead on trauma informed physical healthcare within the setting and join us in helping us achieve positive outcomes for the children placed with us.

    The post holder will be part of the multi-disciplinary psychology Led Healthcare In-Reach team to provide evidence based physical health assessment and treatment to young people aged between 12-17 in a secure welfare setting, as part of a seven day a week rota service.

    The post holder will assertively engage with young people through direct work as well as with the young person's local and wider professional network to identify and manage physical health needs of the young person.

    The post holder will act as the health professional responsible in ensuring good physical health for the young person and improving future long term health outcome.

    Provide weekly health clinics for young people to assess, treat and refer acute
    Provide management plans and patient education around long term health issues as
    Lead on physical health within the secure unit.
    Advise on issues of policy around physical health needs of the young people.

    Engage with outside agencies to enable young people to access full health care provision as every other young person in the area.

    Provide in house education and clinical guidance on physical health issues for the onsite health team.
    Develop protocols and procedures for the in reach health team around physical health interventions.
    Promote the work of the health team in the secure unit within CPFT and to external organisations.

    Work with the lead for mental health provision to create a holistic health focus for the young people within the secure unit.

    Work across teams within the secure unit to promote healthy lifestyle initiatives and create opportunity for the young people to make better health choices.

    Registered Nursing Qualification (General and/ or Children's)
    NMC Registration
    Post Grad advanced practice course/s
    Post-Registration experience at Band 6 or above
    Experience of working with Children, Young People and Families
    Safeguarding experience
    Phlebotomy (or willing to complete training)
    Advanced assessment and care planning of physical conditions
    Ability to develop systems and processes to monitor and improve patient care and services
    Ability to chair meetings and lead discussions with senior staff and external agencies
    In depth knowledge of the physical health care requirements for this role
    Demonstrates experience of clinical risk management
    Occupational Health cleared to perform the duties of the role
    Ability to travel across the Trust site, when required
