    Job summary

    The post holder will support the Clinical Director to ensure the smooth operational running of the PCN and will be involved in the management and development of the network.

    The role of PCNs in primary care and integrated neighbourhood healthcare is continually expanding, to adapt to this growth and align with the evolving landscape, we are expanding our teams to ensure we have the capacity to support our PCNs in delivering effective services for our patients, in partnership with our practices.

    The role is key for our ambitions and is part of a wider PCN support team of ten people.

    This is a part-time role working 34 hours per week.

    Salary Range: £27, - £33, (full-time equivalent) or £ -£ p/h

    Job responsibilities

    The PCN Operations Team Lead forms an integral part of the PCN leadership team working alongside the Clinical Director, Digital and Transformation Managers, Strategic Digital and Transformation Managers and member Practice Management teams, playing a pivotal role in the delivery of high-quality care within the PCN.

    The role holder will be responsible for PCN operational matters, including delivery of the network DES and IIF achievement, line management of PCN staff at sites, production of performance and quality reports, and for oversight and delivery of the network financial processes.

    Working in collaboration with member practices, the post holder will support operationalisation of the PCN strategy (specifically new models of care), provide hands-on operational support to practices, their teams and PCN staff.

    The post holder will support the Clinical Director to ensure the smooth operational running of the PCN and will be involved in the management and development of the network.

    The PCN Operations Team Lead will work flexibly with member Practice Management teams, providing support as necessary, and will take an active role in the development and embedding of the PCNs culture, values, and reputation, as providers of high-quality services.

    DUTIES AND AREAS OF RESPONSIBILITY

    Operational Delivery:

  • Review and update PCN executives and member practices on service delivery, project timescales and KPIs on a regular basis, utilising data and information available.
  • Line manage site based PCN staff.
  • Support the recruitment, induction, training and retention of new and existing staff.
  • Ensure the allocation of resources across the PCN is fair and equitable.
  • Where appropriate, liaise with other Lakeside Healthcare functions, such as HR, Project Management, Group Operations and Finance, to support implementation and roll out of services.
  • Act as a key point of contact for PCN matters for internal and external stakeholders.
  • Work with Digital and Transformation Managers, Site managers of member practices, and Clinical Directors to support designing and implementing project/service delivery plans in relation to the PCN Network Contract Directed Enhanced Service (DES) agreed requirements.
  • Support Digital and Transformation Managers in the development of existing and new SLAs, Project Implementation Plans, and collaboration agreements to meet PCN needs.
  • Support the introduction of new services through providing and maintaining action logs, risk registers and programme updates throughout implementation.
  • Collaborative and Partnership Working:

  • Support the development of PCN partnership and collaboration networks and to develop trusted joint delivery relationships.
  • Where appropriate, work with provider organisations to deliver agreed services to meet PCN SLA requirements.
  • Where appropriate, with provider partners, ensure that agreed tasks and activities are implemented in a timely fashion to an agreed plan.
  • Where appropriate, work collaboratively with the Integrated Neighbourhood Teams to deliver and support key projects in the community.
  • Work innovatively to support the PCN Executive team, Clinical Director and Digital and Transformation Managers, to understand overall performance against key objectives and priorities.
  • Maintain constructive relationships with a broad range of stakeholders, internal and external.
  • Participate in relevant internal and external working groups, projects, and services to provide relevant information and analytical advice to the PCN Digital and Transformation Manager, PCN Clinical Director, PCN Support Team, Project Manager and the HR and Finance functions.
  • Communicate information and issues, including briefings and reports, to member practices and their teams.
  • Compliance, Information Management and Reporting:

  • Identify and process operational data to support operational performance and improvement.
  • Oversee a programme of audits and surveys, that support and evidence measures improvement.
  • Working with member practices, develop, implement, and manage support functions covering business continuity, planning, tracking, reporting, quality management and internal communication.
  • Prepare, check, and submit accurate submissions for Enhanced Services and ARRS claims and any other contractual requirements on behalf of the Network and ensure that all claims are submitted within set out timeframes.
  • Provide all necessary financial information to the Finance Manager on a timely basis, including any anticipated new lines of income or expenditure.
  • With support from the Finance Manager, ensure that the PCNs physical and financial resources are managed and maintained for maximum effectiveness.
  • Manage all delegated budgets with propriety, in line with all relevant organisational procedures, with a view to achieving the best impact from well managed finances.
  • Working with the Clinical Director and Senior managers, maintain appropriate and required Data Sharing agreements/Information Governance policies for services across the networks.
  • Identify and record risks and issues, developing contingency plans with service and contract leads.
  • As part of the implementation and delivery approach, support the PCN to adopt Lakeside Healthcare systems and processes and ensure they are embedded in new ways of working where appropriate (Radar and Rotamaster).
  • Knowledge, Planning and Organisational Skills:

  • Support the development and maintenance of PCN delivery and implementation processes and plans in line with best practice.
  • Support the sharing of and enable the adoption of best practice across the four Lakeside PCNs and provide coordinated and cohesive support in its delivery.
  • Work collaboratively to define plans for the PCN, based on a timetable of delivery and implementation requirements.
  • Support the Clinical Director and Digital and Transformation Managers to develop resource plans, that underpin new integrated care provision with the PCN.
  • Ensure delivery and implementation is against agreed objectives, achieving quality outcomes.
  • Maintain confidentiality at all times.

    Qualifications

    Essential

  • A-level or equivalent experience.
  • Desirable

  • Degree level education or equivalent experience.
  • Recognised project qualification such as PRINCE2.

  • Previous experience of working in an operational role that has included partnership and collaborative working.
  • Line management and team management.
  • An understanding of supporting delivery against existing and new service level agreements and contracts.
  • Knowledge of operational implementation and delivery methodologies.
  • Evidence of continuous professional development.
  • Desirable

  • Experience of working in the NHS, particularly in primary care.
  • Knowledge and experience of working with GP Practices, ICB's, local authorities or GP Federations.
