    Job summary

    You will be joining a large team of behaviour change specialists in ABLHealth including specialist lifestyle coaches, nutritionists, dietitians, GPs,psychotherapists and exercise specialists to deliver evidence based, holisticand realistic weight management to people in the community. Outside of ABL, youwill be part of the CYP INTs, collectively delivering proactive earlyintervention activities aimed at reducing health inequalities focused obesityrespiratory related conditions and mental health.

    You will understand challenges and barriers that Children, Young Peopleand Families face with relation to their health (both physical and mental) andwellbeing. You will engage with and build a good rapport with Children, YoungPeople and Families from varying communities across Bassetlaw.

    Main duties of the job

    You will be expected to engage with all secondary schools acrossBassetlaw, working with staff within the schools and other system partners( Healthy Families Team, Bassetlaw Place Based Partnership) to devise andcoordinate an in-reach programme that will raise awareness of lifestylebehaviors and encourage referrals into existing weight management services. Theideal candidate will be passionate about promoting healthy lifestyles anddelivering motivational change which is client centered. You will have abackground in health improvement and/or nutrition, exercise, public health,community health or adult and/or family weight management and be an excellentteam player and have a willingness to learn and develop.

    Job responsibilities

    Job Purpose:

    Working as the CYP Health & Lifestyle Coordinator within the Your Health Notts service as part of a wider Start Well CYP Integrated Neighbourhood Team (INT), in collaboration with the NHS and wider partners, you will be working with children, young people in secondary school and community settings, to motivate and advise thosewho are currently overweight and need support to improve self-management of asthma through lifestyle changes the Bassetlaw INT is a new innovative development that will deliver targeted interventions to CYP focused on mental health, respiratory related conditions and weight management. As part of this team, you will align workstreams targeting childhood obesity and asthma self-management through supporting the development of bespoke interventions and utilization of existing CYP asthma self-management tools. You will also work with Bassetlaws Primary Care Networks to facilitate the identification of eligible CYP through case finding and pathway development. You will also be responsible for the direct delivery of some projects and must be passionate about working with individuals to begin the life changing processes necessary for permanent and continued weight loss and weight maintenance.

    Experience of working within the community is desirable but not essential as training will be given to develop appropriate skills. The ideal candidate must be a confident practitioner who is able to work autonomously and can make appropriate decisions and judgements in a school setting.

    Duties and Responsibilities

  • To assess, plan, implement and evaluate care.
  • To ensure the provision of effective, efficient, appropriate, high quality patient care.
  • Engage with secondary schools, INTs, PCNs and other key stakeholders in Bassetlaw to develop a school-based programme to raise awareness of lifestyle behaviours and increase referrals to weight management services.
  • Lead on the delivery of multi-disciplinary assessments working closely with ABLs GP and Psychotherapist to determine an appropriate, person-centred care plan.
  • Deliver evidence-based education sessions to raise awareness around nutrition, physical activity and behavioural change as advised by ABL Health team and in line with ABL Health programme curriculums.
  • Support children and young people within the school on a 1:1 basis, facilitating a series of My Story conversations.
  • Work alongside PCNs to ensure those CYP identified through targeted case finding are referred and signposted effectively to current and new provision locally.
  • Contribute to the contract performance report by submitting regular case studies to the Head of Service.
  • Keep detailed files, recording data accurately, ensuring all forms and other documentation is completed correctly.
  • Undertake and complete all mandatory and additional skills training as required.
  • Have a thorough understanding of ABL Health policies and procedures.
  • To attend and actively participate in team and wider company meetings, such as MDT and external meetings such as the Bassetlaw HIF, INT meetings and other relevant forums.
  • Be able to organise own workload in order of priority and deadline dates, daily administration tasks and other work-related activities.

  • Experience of delivering health promotion / public health interventions.
  • Experience of working within a multi-disciplinary team.

  • Experience of delivering groups and/or one-to-one specialist weight management interventions to children, young people and adults.
  • Experience of planning and delivering appropriate health interventions specific to individual requirements.

  • Experience of engaging with a wide range of stakeholders in public health, education and community settings.
  • Experience of delivering health/weight interventions to children and young people.
  • Experience of delivering parenting programmes.

  • The ability to travel independently across the organisational footprint (Bassetlaw).
  • Work flexibly and adapt to suit service need including evening and occasional weekend work as required
