An ideal opportunity has arisen to be part of a well-established Community Matron and Community Nursing team in Reading

  • The post holder will promote shared decision making with individuals regarding their care plan and encourage proactive selfmanagement of longterm conditions with the aim of avoiding crisis resulting in unplanned hospital admission
  • The post holder will work closely with the Community Nursing Teams to manage complex patients as well as the wider multidisciplinary team in a variety of settings to improve health outcomes and quality of life for patients with multiple chronic diseases.
