Support Planner27 per hour umbrella
Contract (3 months+)
You will be part of the Disabled Children's Service, support planning for disabled children and young people who have a Social Worker supported by the Disabled Children's Social Work Team.
This also include supporting social workers to further develop their practice skills regarding support planning for disabled children through modelling good practice.
Your role will be and devise to assist and support the Disabled Children Team social workers to develop support plans in line with needs, goals and outcomes identified in C&F single assessments and CIN Reviews using personal budgets.
You will also work closely with members of the Direct Payments Team to ensure families are supported and advised to access employment advice and advice regarding direct payment arrangements and pre-paid cards.
Experience of working and communicating with disabled children and their families and an understanding of the barriers they face in a culturally diverse urban environment where there are significant deprivation issues.
