Description
Are you passionate about providing an excellent service to our patients?

Do you want to make a difference to patients being discharged from hospital?

Are you looking for an opportunity to positively impact patients' and their relatives' experience of post-inpatient care?

If the answer is yes, then the Patient Flow Coordinator role is for you.

You would be joining a dynamic patient-centred team, providing an excellent service to our patients and ward colleagues.

Working within a supportive and friendly environment, you will be encouraged to participate and share learning with colleagues across the team.

We are looking to expand our Hospital Discharge Team and have a number of Patient Flow Coordinator roles available to support patient discharge.

We are looking for nurses or AHPs to join our team to help continually improve the care given to our patients as they are about to leave hospital.

We know that a patient's recovery can be impacted by staying in hospital longer than is necessary so having the right care plan in place is critical and supports them to leave hospital safely.

This is a really exciting opportunity to make a difference to patients.

Patient Flow Coordinators will support the effective management of patient discharge from hospital and will play a pivotal role in supporting ward-based colleagues with patient discharge plans.
