    We are looking for an experienced Band 6 Occupational Therapist to join our well established Integrated Adult Community Rehabilitation Therapy Team.

    Basildon and Wickford ICT community service is a forward-thinking and innovative service that works in a fully integrated way, with physiotherapists, occupational therapists, district nurses and community matrons all working side by side to ensure an integrated and holistic approach to the people accessing the community services.

    Our community rehab team assesses housebound adults within their home environment. By assessing people within their home environment, we are in a good position to significantly improve people's quality of life and promote independence at home by providing therapy as well as by jointly working with occupational therapists, the other members of our integrated team and having close links with social services ,therefore making this a very rewarding job.

    Main duties of the job To manage a defined caseload of patients with complex needs, using evidence based practice, Occupational Therapy goals and patient centred principles.

    Responsible for:
    • Assess, plan, implementation and evaluation of interventions as an autonomous practitioner.
    • Working as part of the Integrated Community Team ensuring an MDT approach to goal setting and care delivery.
    • Leadership of junior, support and assistants staff through supervision, training and appraisal.
    • Ensuring a high quality of service delivery and treatment/care planning.
    • Improve standards of care by efficient and effective use of resources.
    • Work in accordance with guidance set out in national and local guidance e.g. The NHS Improvement Plan, Supporting Patients with Long Term Conditions, the NSF for Older People.

    Probationary Period

    This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

    Starting with NELFT

    NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

    High Cost Area Supplement - London

    This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of 1,192 and a maximum payment of 2,011 per annum pro rata).

    Job description Job responsibilities Specific Tasks directly related to the post:
    • Maintain an awareness of current developments within field of practice, disseminate information, and ensure that practice is based on the best available evidence.
    • Participate in the Clinical Governance programme, encouraging others in the team to participate also.
    • Participate in any Clinical Audit programmes and the evaluation of own practice within the clinical area.
    • Utilise and participate in research relevant to service.
    • Demonstrates increased knowledge and able to assist in the implementation of innovative evidence based, cost effective clinical practice.
    • Assist in the monitoring and protecting groups and individuals whose health and well-being may be at risk vulnerable adults.
    • Ensuring service delivery adheres to best practice guidance and quality standards.
    • Ensuring evidence in place for Care Quality Commission compliance for the service.
    • To participate in audit (e.g. Essence of Care) within the clinical area supporting the introduction of a change in practice if indicated.
    • Ensuring organisational policies are understood.
    • Ensure that junior and support staff maintain accurate and contemporaneous clinical records in line with Trust policies and National standards, using current system provided.
    • To adhere to Risk management process in line with Trust Policy and to manage any incident in order to learn and minimise the risk of reoccurrence.
    • To maintain Clinical Governance Standards through audit, research and evidence gathering.
    • Familiar with national, professional and local quality issues relevant to the delivery of care.
    To be accountable for maintaining and monitoring measurable standards of care.

    • Current HCPC Registration

    Experience Essential
    • Three years post graduate experience in health, social care or related setting
    • Assess, plan, implement, evaluate OT interventions

    Knowledge Essential
    • Knowledge of basic equipment and adaptations
    • Able to adapt clinical knowledge and experience to various settings
    • An awareness of national current policy affecting health service delivery
