    Job summary

    This role will support the coordination of care for cancer patients through their cancer pathways from pre-diagnosis to end of treatment. This role will be focussed around the telephone support of the cohort of patients who are referred to Frimley Health from local GPs, internal services and other trusts.

    This role will be guided and supervised by the Macmillan Information and Support Officer in collaboration with the Head of Nursing Cancer and Macmillan Information & Support Manager.

    The post holder will coordinate the care and support given to cancer patients, their families and health care professionals who contact the Macmillan Cancer Navigator service at Frimley Health. The remote cancer navigation service will act as a telephone based single point of access for people affected by cancer from pre-diagnosis to end of treatment and support. The post holder will also support recording of national audit data for cancer patients in collaboration with clinical teams to support various elements of the cancer pathway.

    Main duties of the job

    Co-ordinate care by providing a telephone/email based single point of access, for patients, carers and health care professionals from pre-diagnosis to treatment and living with and beyond cancer, and throughout their cancer journey.

    Supporting Cancer Clinical nurse specialists by taking on administrative roles related to supporting Holistic Needs Assessment, documenting, tracking and recording, all patient and service users interactions.

    The post holder will also provide information to patients and service users regarding support available during their cancer pathway.

    Providing holistic support and information and signposting to existing support in the hospital and community. Working alongside registered practitioners to improve care for people with cancer, working closely with Clinical Nurse Specilialists and Allied Healthcare Professionals.

    Job responsibilities

    KEY RESULT AREAS:

  • To make an effective contribution to reaching the Trusts vision, strategic objectives and key work programmes for cancer patient
  • To coordinate care for patients from referral throughout their pre-diagnosis and treatment pathway
  • To provide a touch point for patients and GPs with the hospital to ensure two way communication during treatment
  • To provide guidance, contact and support for patients as they journey through our hospitals
  • To ensure all appointments are notified in a timely manner providing an excellent patient experience providing required information

  • Support people affected by cancer to self-manage as part of SSMP
  • KEY TASKS & RESPONSIBILITIES

  • Development of navigation pathway for patients pre-diagnosis, through treatment and end of treatment
  • Provide a single point of contact for patients throughout their treatment
  • Responsible for contacting and organising appointments and information and results based on decisions made by a registered practitioner,
  • Coach patients and carers in the safe use of any equipment to be used at home in line with guidance provided by a registered practitioner
  • Coordinate face to face appointments as per MDT instructions
  • Comment on proposed changes to policies and procedures
  • Support and contribute to audit processes, governance, research, clinical research trials and service development
  • Report any protocol variance to MDT and CNS
  • Responsible for validation and updating of database(s) required for national and local reporting
  • Support the planning/inviting/organising of informative events for patients
  • The post holder will have a good knowledge and understanding of patient pathways within cancer, including the diagnostic staging investigations and treatment options available.
  • The post holder will have excellent communication and organisational skills, together with a high attention to detail and levels of accuracy.
  • To act as centralised point of contact for patients, receiving, triaging and responding to face to face, telephone and written queries, liaising and escalating to clinical team/ operational teams as required.
  • Demonstrate effective management of time and resources with regards to the role, and within the wider team resources.
  • Respond to unpredictable requests from the Trust, ICS, cancer alliance and regional team to provide information regarding patient pathways and ways of improving this.
  • To contribute in the development of the role by identifying and proposing ways in which the service could better support pathways.
  • Be responsible for day to day office/ home working administrative functions including raising orders of stationary supplies, scheduling of calendar activities, cover for the service during AL/ absence, and any other administrative function as deemed appropriate.
  • Support Trust initiatives to ensure efficient and effective use of resources to maximise utilisation of resources, minimise duplication and variation, and ensure minimal wastage.
  • Contribute to quarterly service reports and end of year report to Trust cancer services and cancer alliance as evidence of the impact of the role on the cancer pathway.
  • Participate and support evaluation programmes of the cancer navigator role, including mentoring for new post holders.
  • Be a single point of contact for patients, carers and the healthcare team triaging sensitive enquiries and referring to the appropriate department within the hospital. Using own judgement as to what requires immediate action
  • Care coordination and navigation of care and support

    Under the guidance and supervision of a registered practitioner:

  • To coordinate the follow up investigations required post cancer diagnosis or treatment
  • To coordinate a rapid re-entry into the specialist cancer team review as per tumour group protocol
  • Co-ordinate and book appointments to the Cancer CNSs as appropriate
  • To develop and maintain a good professional relationship with patients and with the specialist cancer teams
  • To facilitate access to health and well-being services, Moving On programmes and other support services
  • To facilitate the completion of the Holistic Needs Assessment and Treatment Summaries
  • To provide basic telephone advice and refer on or sign-post to other sources of support as per tumour group instruction
  • Facilitate access to third sector support, Macmillan Information Services, Macmillan CAB, JIGSAW, the Fountain Centre
  • To be a single point of contact for patients and their GPs/Primary Care for patients whilst they remain cancer patients at the Trust
  • To support Macmillan Information & Support Officer in the Wexham Park Macmillan centre or at cancer outpatient appointments across all the Trust sites as required
  • Coordination of education and provision of advice

  • Support the organising of focus groups to gather feedback back with advice from clinical staff
  • Support patients and carers to access up-to-date and appropriate information and support, including sign posting to a range of support services
  • Support the direction of the patient or carer to education or support areas. Encourage and support active and healthy lifestyle choices
  • Support patients and carers to understand what signs, symptoms or situations to be aware of that would indicate concern in collaboration with the CNSs
  • Inform patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours
  • Support Frimley Health cancer education programmes by presenting Macmillan Cancer Navigator service with support from Macmillan Information and Support Officer
