    Working within the community empowers me to use my own initiative, working autonomously but also having the support of a great MDT behind me.' - Gateshead CTT are looking to employ Community Mental Health Practitioners across all Gateshead West area.

    We have posts that are suitable for registered Nurses, Social Workers or Occupational Therapists who must be registered with the NMC, HCPC or Social Work England.

    Here within the Gateshead Community Treatment Team there is a real sense of team spirit, and we are looking to add clinicians with this same positive outlook to our already excellent skill set.

    Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.

    We are able to support staff to take up accredited training in evidence based therapies You will be able to develop your skills by working with service users who have a range of complex mental health needs with associated risk, and you will need to demonstrate experience of being able to plan and deliver in collaboration with service users, families and their carers.

    You will need to be clinically experienced, solution focussed and competent in working with individuals, their families, and carers, in a community setting.

    You should have a collaborative and enthusiastic approach as well as the passion and commitment to develop and promote recovery-based interventions for adults with mental health related needs.

    CNTW NHS Foundation Trust will not provide sponsorship for this role.
    12 to 24 month band 5 qualified nurse experience dependent upon service need.
    ~ Experience of working within a community team

    Awareness and understanding of professional accountability, NMC (or equivalent regulatory bodies) standards and other local and national initiatives
    Experience of working with and an understanding of moderate to severe mental health difficulties
    risk assessment and risk management/ safety planning skills
    Evidence of MDT and multi agency working
    Evidence of risk assessment
    Evidence of care planning.
    Proven ability to carryout and provide comprehensive written assessments of individuals with complex mental health needs

    This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
