This is an exciting time for PC24 as we are transforming and implementing new models of care across our practices. We are seeking to appoint Practice Nurses to support our 12 practices and our new purpose-built primary care hub offering a family-friendly service to our patients across Liverpool and Sefton.

About you

NMC Registered nurse with practice nurse experience. Previous involvement in Primary is essential. This role requires consolidation of specialist knowledge and skills in general practice nursing, demonstrating a depth of knowledge, understanding and competence that supports evidenced informed, complex, autonomous and independent decision-making, and care in general practice and related settings. This role will require personal resilience, clinical leadership, supervision and mentorship of others in the general practice nursing team and providing an effective learning environment for staff and students in the wider team. Key requirements of the role you will be required to undertake:

• Vaccinations including baby immunisations.
• Cervical Screening
• Chronic Disease Management
• Confident with QOF

Hours

Primary Care Shifts are available between the hours of 08:00 and 18:30 across all 12 of our locations in Sefton and Liverpool. We are a patient centred organisation and therefore require flexibility to cross cover across cluster sites as required:

Primary Care Hub is based Sefton Road Sefton

Out of Hours face to face shifts vary from hours, 13.00 – 18.00 hours, 09.00 – 17.00, 18.30 – midnight.
