We are recruiting a team of Health Improvement advisors to support our new tobacco dependence treatment programme and help patients to stop smoking.
You will work with different departments to seek opportunities to develop, encourage and implement health improvement interventions, including smoking cessation, across our different sites.
In particular, you will act as a Smoke Free staff champion for the Trust, using timely evidence-based practices to help patients stop their tobacco use both during their hospital stay and after discharge.
You will engage with community-based smoking cessation services to ensure our patients can continue to receive support after they are discharged from hospital.
This post will contribute to tackling health inequalities through promoting and supporting people to stop smoking, as well as looking at other health improvement opportunities such as weight management and alcohol intake.
The role focuses on engaging with patients and offering them practical support to change their behaviour to achieve their own choices and goals.
This role requires an ability to manage time and resources effectively to ensure the team can meet the Trust's standards and targets.
Work with ward staff to build health improvement referrals
Liaise with the pharmacy team about access to pharmacological support for smoking cessation, such as Nicotine Replacement Therapy
Establish and maintain partnerships with local community services to enable an effective transition to post discharge support
To collect data as part of the NHS England's data requirements
To work as part of the multi-disciplinary team to support the wider smokefree agend
