    Job summary

    This is an exciting opportunity to work as part of the newly expanded Liaison and Diversion Service in Cheshire. The post will be working across the footprint of Cheshire supporting the other functions within the Health & Justice service provision.

    This role will include providing high quality assessments for people who are in the criminal justice system. The service will be delivered to peoples who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs etc.

    The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.

    Main duties of the job

    1. Working in a Health and Justice Service in a Cheshire Outreach position responsible for offering high quality clinical service to patients and their families.

    2. Assess patient's individual care needs, agree, and refer on to appropriate agency.

    3. Responsible for clinical and caseload supervision of other team members where identified.

    4. Liaising with multiple criminal justice services and other external partners.

  • Identify, assess, and refer people of all ages with a wide range of vulnerabilities when they are in contact with the justice system.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on service users' presentation and progress.
  • Utilise agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Service.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at a team level.

    The successful applicant will hold a professional registration and have demonstrable experience of working with people presenting with complex mental health and related vulnerabilities.

    You will be responsible for the provision of high quality, holistic assessments to identify any vulnerabilities that the person may present with.

    You will have experience of planning and implementing care plans, engaging with difficult & challenging individuals with complex needs.

    Excellent communication/interpersonal skills.

    The ability to scope out local services and to build excellent working relationships with secondary/tertiary and voluntary sector organisations.

    An ability to offer excellent clinical support to colleagues/students, promoting positivity and supporting innovation .
