What you can expect to be doing:
As our colleagues professionally grow, we have opportunities across our teams.

We are currently looking for a new colleague to join us in our Bournemouth Community Teams supporting people living with long term conditions to live inclusively in the community, alongside supporting their carers, children and families.


When you join this team you will be based at either the Boscombe office or Bournemouth Civic Centre with the ability to work flexibly across locations in the BCP area combined with home working.

You will work alongside a multi disciplinary team of skilled and experienced professionals carrying out holistic, assessments of need and support, coupled with care and support planning and reviewing packages of care.
