This is an exciting opportunity to support vulnerable adults living in their own homes in the community, ensuring that they can stay at home during times of poor health or other emergencies, and to support them, where appropriate, to build their skills and levels of independence back up through the achievement of smart and measurable goals.

This is a valuable service, making a real difference to our local communities while supporting local health services to maintain capacity by reducing the need for people to stay in hospital if they can be supported safely and effectively at home through your caring and nurturing approach.
