    An excitingopportunity has arisen for a dynamic nurse to join the Intermediate Care Teamin First Community Health and Care.

    The IntermediateCare Team (ICT) are an admission avoidance and facilitating discharge service,who provide assessment, short-term care and rehabilitation in the patientshome.

    Excellent communication skills are essentialto enable liaison with all our health and social care partners in the management of patients discharge from hospital and assessment in thecommunity.

    The service runsfrom 8am-8pm 7 days a week, 365 days a year, the post holder will be expectedto work on a rota basis.

    Workingclosely with Physiotherapists, Occupational Therapists and RehabilitationAssistants the post holder is responsible for assessing the health and socialcare needs of patients and their carers within the community, aiming to preventthe need for patients to be admitted to hospital and facilitating theirdischarge home.

    They will act as a role model for junior team members andcontribute to the promotion of an environment which encourages and supportsdevelopment of staff using evidence-based nursing practice.

    They will be expected to utilise a variety ofstrategies to support the patient at home and to communicate effectively withfamily, carers, the multidisciplinary team, voluntary organisations and otheragencies, to facilitate rehabilitation and promote independence.

    Evidence of Continued Professional Development
    Minimum of 2 years post graduate experience
    Experience in line managing junior staff
    Experience in working in a primary healthcare setting
    Good knowledge of health and safety and risk management
    Flexible approach to service delivery
    Good understanding of current nursing challenges
    Clear understanding of health and social care services
