Service care Solution are currently recruiting for a Social Work Assistant with Hackney Council, to work in Adult Services.

PURPOSE OF JOB

To provide support, information and advice and technical assistance to individuals and to develop, cost, negotiate and mediate their individual support plans as required.
To enable individuals to take control of their support plans and resources.
To assess people who are eligible for social care support to develop their own solutions and enhance their quality of life through promoting choice.
To work jointly and also alone to develop support plans with people and implement these to enable people to be healthy, safe and well.

KEY ACCOUNTABILITIES

Using person centred approaches, enable people to develop outcome focussed support plans that meet their identified needs
To support individuals to identify potential risks and find solutions.
To assist people to find creative solutions to meet their identified needs
To present information about the support plan, and where relevant with the service users, to other professionals and relevant panels.
To help people to put their support plans in place in order to meet their needs
To work with all relevant people and organisations in support of a person to ensure their needs are met effectively, responding to timescales as appropriate
To ensure, where necessary that all relevant information is communicated to key stakeholders/providers to ensure a person's needs are effectively met.
