An exciting opportunity has arisen for a dedicated and enthusiastic Substance Misuse Practitioner to take a lead role in providing the successful and effective delivery of Substance Misuse services and Dual Diagnosis in Clayfield's Local Authority Secure Children's Home.


You will be joining as a Dual Diagnosis nurse within a multi-disciplinary Health Team in a secure Childrens home setting.

Providing care to young people with complex substance misuse and mental health problems, from the point of entry into Clayfield's House to re-entry into the community so that offending is reduced, and the health and social functioning of young people is improved.


A high-quality patient care will be delivered through assessing, planning, implementing and evaluating care for young people with substance misuse and mental health co-morbid problems working to both harm reduction and health promotion / recovery agendas.


This post will support the Health Team in delivering an effective multi disciplinary team approach to the care of Young people within Clayfield's House.

To provide expertise and contribute to substance misuse being systematically identified and responded to in a timely, evidence-based manner.


To implement systems to ensure all service users are being assessed for alcohol and drug use as part of a comprehensive assessment and information is recorded in line with Trust standards.

To provide specialist assessment and clinical formulation to clients with complex mental health needs and co-occurring substance misuse.

To provide expertise and contribute to development of effective care plans to support patients with co-existing substance misuse/mental health


Forge strong links with partner substance misuse services - it is expected that the post holder will spend a proportion of their time working within partner agencies to deliver training, advice and consultation in relation to mental health.
