    Job summary

    Park Surgery in Horsham, WestSussex has an opportunity for an 6- 8 session salaried GP. Located in the town centre with on-site parking available.

    We are looking for a dedicated professional tostep into this role and thrive within our large busy practice and team. We can offer a flexible/hybrid model with the option of a term time contract. We encourage and support profession development and training in special interests.

    Horsham is an excellent place towork, being an attractive market town surrounded by beautiful Sussexcountryside.

    We are a large practice with 23,000 patients and supportinginfrastructure to match. The team comprises 6 partners, 3 salaried GPs, 3ANPs, 4 PAs, 3 Clinical Pharmacist, 6 Practice Nurses and 3 highly experienced HCAs. We have an extensive PCN workforce consisting ofClinical Pharmacists, Paramedics, a Pharmacy technician, Social prescribers, CareCo-ordinator's, a 3 First Contact Physio and a Physicians Associate.

    Main duties of the job

    Provide high quality medicalservices to patients on a personalised list and other registered patients withthe practice including any administration connected with those services.

    Provide a full range of servicesin line with the requirements of the GMS Contract and Enhanced ServicesContracts. Refer patients as appropriate and in accordance with localprocedures.

    Undertake duties such asprescription signing, prescribing reviews, telephone advice, triage, and late doctor in accordance with the rota.

    Job responsibilities

    Primary Responsibilities

    The following are the core responsibilities of role.There may be on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:

    a. The delivery of highly effective medical care to the entitledpopulation

    b. The provision of services commensurate with the GMS contract

    c. Generic prescribing adhering to local and national guidance

    d. Effective management of long-term conditions

    e. Processing of administration in a timely manner, includingreferrals, repeat prescription requests and other associated administrativetasks

    f. On a rotational basis, undertake Late Doctor roles once amonth.

    g. On a rotational basis, undertake Extended Hours 2 Hrs amonth.

    h. Maintain accurate clinical records in conjunction with goodpractice, policy and guidance

    i. Identify, implement and embed best practice through clinicalguidelines and the audit process

    j. To take responsibility for continuous improvement and qualityinitiatives within the practice

    k. Accept delegated responsibility for a specific area (orareas) or the QOF

    l. Attend and contribute effectively to practice meetings asrequired

    m. Commit to self-learning and continuing professionaldevelopment

    n. Review and adhere to practice protocols and policies at alltimes

    o. Be the named lead clinician for a care home includingcarrying what is required in the Nursing Home LCS

    SecondaryResponsibilities

    In addition to the primary responsibilities, the Salaried GPPartner may be requested to:

    a. Participate in practice audits as requested by the audit lead

    b. Participate in local initiatives to enhance service deliveryand patient care

    c. Participate in the review of significant and near-miss eventsapplying a structured approach root cause analysis (RCA)
