    We have a fabulous opportunity for an enthusiastic Nursing Associate to join our multidisciplinary team consisting of Clinical Pharmacists, Physician Associates, Diabetes Specialist Nurse, Advanced Clinical Practitioners, Trainee Nurse Associate, Dietitian, Physiotherapists etc...

    We are seeking an experienced, self-motivated, hard-working, and patient focused nursing associate to join our team and manage a caseload of patients, making sure that support and care is made available to them and that their complex needs are addressed.

    Main duties of the job

  • To support the organisation and facilitation of clinical care, in a holistic and proactive manner, to patients registered within our PCN.
  • To maintain a high professional standard of nursing care, in adherence to all PCN protocols, policies and guidelines, as well as to provide nursing assistance to the GP's and other members of the primary healthcare team
  • To demonstrate flexibility, in line with the present atmosphere for change within the health service delivery.
  • To work within your own level of competency, and expanding the role in accordance with PCN requirements and own scope of practice

    Wolverhampton North Network is a successful PCN, consisting of 7 practices and over 50,000 patients. The successful candidate may need to be able to travel to all sites and have effective time management processes.

    The applicant can expect to work within a supportive environment where you will receive regular clinical supervision, and will be actively encouraged to undertake further training and study in order for you to continue to develop professionally, in accordance with your agreed personal development plans.

    We will support you in the development of this exciting new role, providing clinical supervision and a named individual to provide general advice and support.

    Job description

    Job responsibilities

  • work as part of the PCNs MDT to provide and monitor care, under direct or indirect supervision
  • improve safety and quality of care at every opportunity
  • contribute to the delivery of integrated care
  • work with the PCN MDT to ensure delivery of nursing associate duties
  • complement existing workforce
  • provide support and supervision to training nursing associates, healthcare assistants, apprentices, and those on learning assignments/placements as required
  • support registered nurses to enable them to be able to focus on the more complex clinical care
  • develop relationships across the MDT to support integration of the role
  • across health and social care including primary care, secondary care, and mental health
  • perform and record clinical observations such as blood pressure, temperature, respirations, and pulse
  • after undertaking additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN, in line with the competencies of the role
  • promote health and well-being to all patients, for example undertaking the NHS health check
  • care for individuals with dementia, mental health conditions, and learning disabilities
  • advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services
  • communicate proactively and effectively with all MDT colleagues across the PCN, attending and contributing to meetings as required
  • maintain accurate and contemporaneous patient health records; and
  • enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.

    Person Specification

    Qualifications

    Essential

  • Registered Nursing Associate on the NMC register
  • Has completed a foundation degree awarded by a Nursing and Midwifery Council (NMC) - approved provider over a 2-year period
  • Has a minimum of GCSE Maths and English at grade 9 to 4 (A to C) or Functional Skills Level 2 in Maths and English
  • Holds a full UK Driving licence and access to a vehicle insured for work purposes is essential for this position
  • Experience

    Essential

  • Experience of working in a multi-disciplinary team
  • Experience of dealing with vulnerable and complex patients
  • Experience of providing, receiving and recording complex, sensitive information
  • Experience of dealing with a wide range of clinical conditions
  • Perform and record observations eg BP, temperature, respirations, pulse Maintain health care records
  • Experience managing long term conditions such as Asthma, COPD, Diabetes, CHD
  • Desirable

  • Experience and knowledge of Primary Care Networks and/or General Practice
  • Experience in using EMIS
  • Experience in Immunisations (National programmes), Cervical Cytology
  • Experience of ECGs, chaperone procedure, flu vaccinations, venepuncture. Experience of dementia, mental health conditions
