    Seeking a highly motivated children's nurse to join our friendly CCN team, someone who is passionate about providing the best care to children, young people and their families within the community setting.


    Providing mainly acute nursing care to children within their homes and other community settings as well as within the acute children's ward, Medical day unit and Paediatric assessment unit.


    Providing training and support to families, schools and carers to enable children and young people to live their lives away from the hospital setting.

    To develop yourself and the service.

    To provide a community service to the local area, supporting the Paediatric Department. Liaising with the MDT and tertiary centres for continuing care

    Provide a community service to children requiring supporting nursing care in the community.

    Liaising with the wider multi disciplinary team and external agencies to support keeping patients at school and within the community.

    Carry out regular oncology and cardiac community visits.

    Administer chemotherapy in the community.

    Carry out wound care assessments and dressings.

    Hold a caseload of patients with long and short term health conditions.

    Provide 24/7 palliative care provision to families wishing to be at home at end of life.

    Working closely with local hospices and services to provide excellent care for the families.

    Training opportunities available.

    Work closely with the inpatient and ambulatory settings along with specialist nurses and the medical team.
    Current paediatric experience.
    Aware of key principles of paediatric care and implications for practice
    Monitors and contributes to protecting children and young people at risk.
    Safeguarding experience
    Ability to travel to community patient visits and carry equipment
    Able to work flexible shifts including weekends and bank holidays and on call as required

    Registered Sick Children's Nurse (NMC Sub -Part 1 (Children) /RN Child Mentorship/teaching course
    ~Community experience Working towards community nursing degree
