    Job summary

    In collaboration with Dementia UK, Havant and WaterloovillePrimary Care Network (PCN) is looking to recruit a registered nurse (RGN, RNM,RNMH, RNLD) with apassion for, and experience of, providing relationship-centred dementia care tojoin the PCN as an Admiral Nurse.

    The successful applicantwill be an NMCregistered nurse, withsignificant post-registration clinical experience ofworking with people with dementia andsupportingtheir carers/supporters, alongwith formal knowledge ofdementia acquired through clinical post-graduatetraining/education.

    Main duties of the job

    Admiral Nurses are specialist dementia nurses who work with and support people with dementia and their families who have complex needs. As an Admiral Nurse you will provide specialist one-to-one support, guidance, and practical solutions for families, alongside supporting best practice and providing role modelling across the will lead Havant and Waterloovilles Dementia Health & Wellbeing Team (a small established team of dementia-specialist Health & Wellbeing Coaches, who will underpin the work of the Admiral Nurse) across the five GP practices within the PCN and will also influence and lead on the delivery of work streams to improve the experience of care for people living with dementia, and their families.

    You will be employed by the Bosmere Medical Practice on behalf of Havant and Waterlooville PCN, working under the management of the Clinical Director for the PCN and will receive ongoing support from Dementia UK to maintain and strengthen your clinical expertise as a specialist nurse within dementia care. You will also become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and develop professionally, as informed by the Admiral Nurse Competency Framework.

    Job responsibilities

    Clinical

    Deliver regular clinical 1:1 sessions for families affected by dementia who meetthe referral criteria within the registered to the five PCN practices

    Provide clinical leadership on the care of people with dementia and their families from peri-diagnosis to post bereavement,to support reduced reliance on GP appointments

    Providespecialist nursing assessment, using the Admiral Nurse Assessment Framework orappropriate tools, for families affected by dementia, including positive risktaking, managing distressed behaviours and risk minimisation strategies

    Providea range of bio-psychosocial interventions to promote health and wellbeing forpeople affected by dementia and their families

    Workcollaboratively with other professionals to improve the identification andmanagement of co morbidities and frailty for people with dementia and theirfamilies

    Developand support care plans for people with dementia and families including positiverisk management

    Supportpeople with dementia, families and staff to understand and respond to changes in behaviour and relationships as aresult of dementia

    Providecare coordination for a clinical caseload of families with complex needs asidentified within triage assessment

    Ensurethe safeguarding of vulnerable adults and that all legal requirements relatingto Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and LibertyProtection Safeguards are considered and adhered to in relation topatient care

    Actas an advocate for people with dementia and their families to promote theirrights and best interests

    Participateand lead on case conferences, continuing health care processes and bestinterest meetings where required

    Workcollaboratively with health, social and voluntary sector services to achievecoordinated care

    Workin partnership with colleagues to promote the use of advance care planning forpeople with dementia and to support and inform families around advance care planning,best interest decisions and end of life care

    Manageand advise on complex care transitions for people with dementia and theirfamilies to reduce the frequency of crisis situations

    Regularlyattend multi-disciplinary team meetings for long-term conditions to inform andreceive information regarding patients who have a diagnosis or potentialdiagnosis of dementia

    Referto secondary care if there is deterioration in condition/a review of medicationincluding antipsychotics is required or any other concerns regarding a personshealth that cannot be managed in primary care, in line with their dementiaexpertise

    Supportand advise on carer assessments and reviews

    Ensurethat care is delivered in accordance with relevant local and national guidelinesand all relevant Havant and Waterlooville PCNs policies

    Recognisethe limits of own competency and professional boundaries and make appropriateand timely referrals to other services where required

    Ensure that clinical records are up to date and maintainedand stored appropriately in line with local and national GDPR regulations

    Educationand Leadership

    Leadand develop the specialist Havant & Waterlooville PCN Dementia Support team

    Facilitate and support the development and delivery ofinnovative dementia training and education to staff, to improve competence and confidence, inline with best practice and guidance

    Participatein and/or lead formal training events promoting best practice in dementia careand disseminating Admiral Nursing work

    Provideprofessional and clinicalleadership and mentorship, acting as a clinical role model on the delivery ofevidence-based practice in dementia care within Havant andWaterlooville PCN

    Adviseand support on the development and delivery of work streams as part ofHavant and Waterlooville PCNsdementia strategy, where appropriate

    Work proactively with key local and nationalstakeholders to develop more integrated care pathways and holistic models ofcare, through dissemination of specialist skills and knowledge

    Contributeto the delivery of local policy, procedures and protocol for Havantand Waterlooville PCNto improve identification,assessment, ongoing support for families facing dementia

    Evaluationand Audit

    Supportthe audit and evaluation of quality improvements and initiatives aimed atimproving the delivery of dementia care within Havant andWaterlooville PCN where appropriate to role

    Collectand review quantitative and qualitative data, as agreed, to evaluate and auditthe Admiral Nurse Service and make necessary improvements

    Undertakeanalysis and evaluation and make any recommendations to the Havantand Waterlooville PCNsDementia Strategy Group or similar

    Ensureservice information and data recorded is relevant, accurate, complete andcaptured in a contemporaneous manner

    Supportevaluation of workforce development in dementia to improve staff knowledge andskills across Havant and Waterlooville PCN

    Liaise withDementia UK Insights and Evaluation team where appropriate, to ensure that theimpact and effectiveness of Admiral Nurse interventions are well evaluated

    Befully aware of and comply with the Data Protection Act and the GDPR inrelation to protecting the confidentiality of patient and service-userinformation and enabling appropriate information-sharing

    Professional

    Ensure all nursing practice is carried out inaccordance with the agreed policies and procedures of Havant and WaterloovillePCN and the NMC Code

    Attendand actively participate in practice development, clinical supervision, andcontinuing professional development, including the Admiral Nurse Competencymodule and the Admiral Nurse Forum, as supported by Dementia UK

    Developown competence through use of the Admiral Nurse Competency Framework andportfolio development using resources provided by Dementia UK

    Maintainprofessional registration in line with NMC guidance, including re-validation

    Contributeto and lead the development of policies, procedures, and protocols relevant todementia and Admiral Nursing within Havant and Waterlooville PCN includingthe Standard Operational Policy

    Ensurecompliance with Havant and Waterlooville PCNs clinical governance requirements,including all mandatory training such as CPR, Safeguarding, Mental Capacity Actand Deprivation of Liberty Safeguards

    Participatein clinical supervision/mentoring within Havant andWaterlooville PCNwhere appropriate

    Maintainup-to-date knowledge of research and evidence-based recommendations onsupporting people with dementia and their families within the primary caresetting

    Qualifications

    Essential

  • Registered Nurse (RMN/RNMH/RGN/RNLD)
  • Post-registration qualification / training in dementia
  • Post-graduate training/ education in dementia/older people
  • Willingness to work towards continued professional and practice development through Dementia UKs Admiral Nurse Academy, including the level 6 or 7 Admiral Nurse Competency Framework Module.
  • Willingness to complete a health coaching qualification as outlined in the NHS England NHS Improvement Implementation and Quality Summary Guide for Health & Wellbeing Coaching
