    An exciting opportunity has arisen for a Band 5 registered nurse to work within a Community Treatment Team based in Gateshead.

    The Gateshead West Community Treatment Team is a multidisciplinary service which provides assessment and treatment for people experiencing moderate to severe mental health problems.

    Your role will incorporate working closely with partner agencies/services in delivering effective care and treatment.

    Whilst working in the team you will have access to a bespoke training programme which supports clinicians in this field to learn and develop.

    The successful candidate will be responsible for the assessment, and treatment of service users who have a variety of complex needs, diverse presentations, and a range of mental health conditions.

    In collaboration with the Service User and their families, the post holder will facilitate formulations which will inform Recovery and Wellbeing focussed collaborative Care Plans.

    Ensuring regular reviews to monitor progress and adapt the Care Plan as necessary.

    They will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting resilience, recovery and wellbeing and maximising independence.

    Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions
    First Level Registered Nurse- mental health or learning disabilities
    Complete preceptorship programme for newly qualified nurses
    Further training relevant to the post
    Experience during training or post qualification working in a mental health setting
    Experience working in the community setting
    experience of working with adults with a mental health condition

    This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
