Byron & Arnold & Calverton Primary Care Networks (PCN) is recruiting Social Prescribing Link Workers due to setting up a new service to run within their GP Practices.

We have a person-centred approach in connecting people to community groups and statutory services for practical and emotional support.

Link Workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.

You will be part of a wider team of Social Prescribing Link Workers working in other Nottinghamshire PCNs and supported by our Senior Social Prescribing Link Worker & Social Prescribing Manager

Byron PCN has a patient population more than 38,000 and covering the following GP Practices:

  • Oakenhall Medical Practice, NG15 7UA
  • The OM Surgery, NG15 7JP
  • Torkard Hill Medical Centre, NG15 6DY
  • Whyburn Medical Practice, NG15 7JE
Arnold & Calverton PCN has a patient population more than 33,000, and covering the following GP Practices:

  • The Calverton Medical Practice, NG14 6FP
  • Highcroft Surgery, NG5 7BQ
  • Stenhouse Medical Centre, NG5 7BP

Main duties of the job:


KEY RESPONSIBILITIES

  • To take referrals from a wide range of agencies and work with your own patient caseload.
  • To provide personalised support which includes meeting people on a onetoone basis, including home visits, to help patients work through issues affecting their health and wellbeing.
  • To build strong links with local VCSE organisations, community and neighbourhood level groups to create a map of community groups and assets available to patients.

KEY REQUIREMENTS

  • NVQ Level 3, Advanced level or equivalent qualifications or working towards this level.
  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement.
  • Experience of supporting people, their families and carers in a related role (including unpaid work).

Job responsibilities:


JOB PURPOSE


Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical link workers who give time, focus on what matters to me and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

Link Workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.


Social prescribing can help to strengthen community resilience and personal resilience and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities.

It particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.


KEY RESPONSIBILITIES

  • Take referrals from a wide range of staff working within GP Practices and multidisciplinary teams within Primary Care Networks
  • Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes. Develop trusting relationships by giving people time and focus on what matters to me
