    Main duties of the job

    To provide specialistpharmaceutical technical services, professional advice and direct support to the Practice Team.

    To liaise withpatients, clinical and non-clinical staff, practice, hospital and communitypharmacists and patient's carers to improve the care ofpatients.

    To maximise support to all clinical and non-clinical practice staff to improve patient care in relation to all aspectsof medicines and their impact.

    To provide professionalspecialist medicines all health professionals topromote the safe, effective and high-quality, cost-effective use of medicinesin all prescribing decisions for the benefit of patients.

    Job Description Pharmacy Technician

    1. JOB SUMMARY

    To provide specialistpharmaceutical technical services, professional advice and direct support toPractice GPs, Nurses, Pharmacists, HCAs, and Reception/Administrative Staff.

    To liaise withpatients, clinical and non-clinical staff, practice, hospital and communitypharmacists and patient's carers to improve the pharmaceutical care ofpatients.

    To develop the role ofthe Practice Pharmacy Technician to maximise support to all Clinical andNon-clinical practice staff to improve patient care in relation to all aspectsof medicines and their impact.

    To provide professionalspecialist medicines advice to GPs, Nurse Practitioners, Practice Nurses,Community Pharmacists, Community Nurses and other health professionals topromote the safe, effective and high-quality, cost-effective use of medicinesin all prescribing decisions for the benefit of the Practice's patients.

    2. DUTIES AND RESPONSIBILITIES

    To act as a TechnicalPharmacy Advisor to Practice Clinicians

    Undertake patientfacing/supporting work to ensure effective medicines use through shareddecision-making conversations.

    Carry out medicinesoptimisation tasks including effective medicine management, supportingmedication reviews and reconciliation.

    Provide specialist expertiseto address the public health and social care needs of patients to help tacklelocal health inequalities.

    Support initiatives forantimicrobial stewardship to reduce inappropriate antibiotic prescribing.

    Support theimplementation of national prescribing policies and guidance, contributing tothe Quality and Outcomes Framework and enhanced services.

    To advise PracticeManagement and other non-clinical staff concerning administrative aspects ofmedicines management as appropriate

    To manage repeatprescription requests, resolving queries where possible and producingprescriptions for clinical review and signature

    To address thefollowing areas without the need to consult a GP/Clinical Pharmacist in eachcase (although clinical support and advice will always be available ifrequired):

    Review dates followingmedication reviews

    Facilitate and documentcorrect addition & removal of medications following receipt of a dischargesummaries, secondary care and community clinic letters.

    Tidying repeatmedication lists including synchronising durations and removing medicationsnot ordered within a defined period

    Changing acutemedications to repeat where appropriate.

    Co-ordinate medicaldevice prescription requests, including requests for catheters, nutritionalsupplements/feeds and stoma care products.

    To work in conjunctionwith the nominated Clinicians who support Nursing/Care Homes in the area.

    Provide independentinformation to patients with regard to medicines and prescribing changes,initiating further support from other healthcare professionals whereappropriate

    To review practice stock management anddevelop systems to improve processes.

    Analyse Practiceprescription data to produce reports as requested

    Participate in reviewand setting of policies relating to medicines management and prescribing

    Support and influencethe Practice Clinical, Management and Administrative Teams to implementprocedural and service changes relating to prescribing to ensure best practiceis adopted in accordance with local guidelines, legislation, directives, andsafety updates , NICE, MRHA etc.

    3. SUPERVISION RECEIVED

    The postholder willbe operationally line managed by the Practice Pharmacists.

    4. SUPERVISION EXERCISED

    The postholderhas no day to day line management responsibility and does not directly manageany members of staff. However, they are responsible for mentoring andsupervising the Prescription Clerks.

    Person Specification

    Qualifications

    Essential

  • Registered with the GPhC
  • Enrolled in or have completed the PCPEP or MOCH.
  • Skills and Knowledge

    Essential

  • Excellent written and verbal communication skills
  • Knowledge of pharmacy procedures
  • Understanding of medicines legislation and ethics
  • Knowledge of diversity issues and ability to work in non-discriminatory way.
  • IT skills including MS Word and Excel
  • High levels of accuracy and attention to detail
  • Desirable
