The post holder will be part of a dynamic team of clinicians whose responsibility is to deliver care to its practice population who are experiencing common mental health problems.

Post holders will have a high level of autonomy within the primary care setting, developing and managing all aspects of their service delivery, according to practice population need.

The role is to provide mental health screening assessments, risk assessment, telephone triage, sign posting and referral to other services and brief intervention for people experiencing common mental health problems within the primary care setting.

The post holder will support GPs and the wider multi-disciplinary team.


The post holder will also provide support, education and problem solving advice to other members of the practice team particularly in the areas of assessment and management of mental health problems.

They will work under the supervision of a designated clinical lead. Principle Responsibilities Assess individuals presenting with mental health needs Work in partnership with service users to identify effective strategies to promote understanding and management of presenting mental health problems Provide information, advice, education and health promotion for service users and carers, about all aspects of mental health and the options, choices and opportunities available to them To signpost and support referral of service users to a range of services, as appropriate including primary, secondary and voluntary agencies and to follow up where required Where identified through assessment, facilitate the stepped care model/clinical pathways for clients experiencing enduring mental health problems To attend and activity participate in the multidisciplinary primary care team meetings weekly in the practice To develop and maintain meaningful and effective working relationships within GP practice setting and the wider Primary Care Network Promote the role of the primary care mental health nurse, as well as the understanding of the role of other health and social care professionals in the primary care location Maintain up to date IT skills and knowledge to ensure effective use of computer systems within primary care To work alongside the social prescribing team to maintain up to date and accurate knowledge of local resources available relevant to meeting service user mental health needs To ensure that appropriate safeguards and practices are adopted when working autonomously with clients independently including the keeping of accurate up-to-date records and data collection To ensure that work is undertaken within the aims, objectives and philosophy of the team and of the wider organisation To be Individually responsible for respecting and promoting issues of equality, diversity, and rights in accordance with good practice and legislation To undertake mandatory training as required To support the practice team in the identification of the mental health training needs of primary care staff To keep abreast of relevant research based practice, to participate in any research undertaken within the practice as appropriate and to share developments, personal skills, knowledge and experience with other team members
