A fantastic opportunity has arisen for an enthusiastic pro-active person to work as a Discharge Support Assistant within the Specialist Medicine Discharge Team.

We are looking for someone who is able to work on their own initiative as well as part of the great team.

support the wards to discharge their patients in a timely manner.
As the discharge support assistant the roll is to improve outcomes for patients being discharged once medically fit.

Early identification of potential needs and additional support is very important in achieving a short length of stay and reduces risk of potential readmission.

An ability to demonstrate trust values and behaviors. Have relevant previous hospital experience and a desire to progress in health care. A good working understanding of NerveCentre.

Will have or be willing to work towards completing training that enhances knowledge and skills. Have a flexible approach to meet the needs of the service.
