    Summary of role:

I am an 9 year old girl living at home with my mum, dad and 3 sisters.

I am a very social person and love being around people. I like to do the same things most 9 year olds like to do, but I need support with those activities.

I like playing in my garden on my swing and trampoline. I also like getting out and about to places like the park, the swimming pool, soft play centres and the shops. I enjoy going on my bike, and I enjoy trying new experiences and challenges. I also like quiet time and watching TV, reading books, jigsaws etc.

I have diagnosis of cerebral palsy. When I'm indoors I can walk around independently, but when I am outdoors I may need more support. When outdoors on hard surfaces I would need my PA to hold my hand. This is for safety and to stop me falling over and potentially hurting myself.

I have epilepsy and I may need lifesaving medication for when I do go into a seizure. Training will be provided to be able to deliver this.

I use different ways to communicate. I have some words but mainly use Makaton and my communication device. I would like my PA to use Makaton and help me with my device - training can be provided.

    Summary of Duties:

I would like my PA to be fun, kind, patient, and energetic, be able to support me with the things I find difficult to do and be a good communicator.

    · To provide some support to the young person at mealtimes e.g. cutting up food/ helping with drinking.

    · To support the young person to work on independent living skills.

Candidates must be able to drive with a clean driving licence and access to a car (preferable).
