    We are seeking an experienced and dynamic Occupational Therapist who is passionate about community working to join our Hospital and Community Team, based at the Gorton Hub.

    This is an exciting new post for someone who is person centered in their practice and who has a particular interest in supporting elderly, frail patients to progress and remain at home following hospital.

    The Hospital and Community Team is well-established team, consisting of physiotherapists, occupational therapists, and rehabilitation assistants. You will work with a wide variety of patient presentations, including those with multiple pathologies and complex needs.

    This is an exciting opportunity to work collaboratively alongside other community teams within Central Manchester, particularly the Discharge to Assess Team to support patient flow and hospital discharges.

    You will provide ongoing support and short- term community rehabilitation to patients, following their hospital discharge aiming to assist them to reach their desired potential in their home environment.

    You will work closely with acute therapy colleagues at Manchester Royal Infirmary to facilitate hospital discharges for patients with more complex manual handling and seating needs.

    You will continue to support and follow up these patients into the community.


    You will benefit from working in a friendly team with experienced staff who are committed to high quality patient care and undergraduate education.

    You will supervise/support more junior staff including students and provide clinical and managerial support to MDT colleagues.

    You will also be responsible for leading and developing aspects of the service in agreement with senior staff and you will be supported with learning objectives as part of an established appraisal process.

    Our core hours are currently Monday to Friday, 8am-4pm. The post is full time, however, part-time may be considered.

    Degree in Occupational Therapy

    Relevant post graduate experience in variety of specialities at band 6/7 level
    Experience with undergraduate education

    Community experience
    Experience and interest in manual handling risk assessments to support patient pathways
    Ability to demonstrate a sound understanding of clinical governance and risk
    Access to a car for business purposes to enable travel between sites and to patients' homes
