Patient Safety To support teams to continuously improve patient safety The post holder must be able to demonstrate a high level of decision making To lead on ensuring a robust process is in place to advise and support managers and staff to manage incidents and investigations both internally and externally as per policy both during normal working hours and out of hours To lead on ensuring that structures and systems are in place within a designated sphere of responsibility so that all incidents are reviewed, investigated and learning fed back in a timely manner in accordance with Trust policy.

To work directly with clinical teams, divisions and specialties to ensure the implementation of the Trust Incident reporting system, providing facilitation of RCAs and investigative support where appropriate to relevant managers responsible for specific areas within the Trust.

To ensure all incident investigations are quality checked and scored in accordance with the Trust incident grading matrix document before uploading to NRLS, reporting to external agencies or data circulation To co-ordinate designated functions of and provide support as required for the management of the Safeguard risk management software system.

To lead and support the Patient Safety Team to review incidents that that been completed by managers and sent for closure/final approval within the system, ensuring data accuracy and quality.


Analyse and critically appraise incident investigation reports and provide constructive feedback to managers in relation to the quality of incident investigation submitted for closure/final approval.

To liaise with the System coordinator around any system related issues, ensuring that the system is regularly updated as new upgrades are released and that staff are trained to use the system.

To support investigations for Incidents / SIRIs within the Trust as required, undertaking agreed root cause analysis methodology to ensure that actions are developed, implemented, monitored and reviewed for effectiveness.

To ensure that the Being Open Policy and Duty of Candour is applied in line with national policy and regulatory requirements and that it is operationalised as part of the SIRI process.


To ensure that communication and systems and processes are in place to support the integrated governance processes with colleagues in Inquests, Litigation and Complaints To facilitate, the identification of risks arising out of incidents are communicated to the relevant managers, governance leads and clinical leaders to enable the risk registers to be effectively populated.

To lead and support internal and external quality and safety inspections and develop appropriate action plans in conjunction with General Managers, Clinical Directors and Matrons and verify the completion of these in a timely manner.

To work closely with the Complaints Department and PALS and Legal Service Team to identify areas of patient safety arising from complaints/ concerns investigations which need to be addressed To work closely with the specialty teams to develop a systematic approach to the communication of lessons learned from incidents and complaints and to support a philosophy Duty of Candour and of Being Open.

To ensure that actions are taken or are in place to deal with immediate patient safety risks arising from incidents, ensure appropriate escalation of serious incidents to the Head of patient safety, Investigations and Inquests or Senior Quality and governance Manager in their absence.

To facilitate the provision of reports and progress against action plans/ lessons learnt with regards to incidents.

To scrutinise SIRI investigation reports against NPSA best practice requirements to ensure that root causes have been identified, that the appropriate conclusions / recommendations have been made and actions planned have addressed the issues.

Facilitate designated groups or committees, ensuring timely circulation of agendas, SIRI investigation reports, and invitation of RCA leads to present their reports to the group.

Liaise with the Commissioners in relation to updating progress against SIRI investigations and actions, providing updates as requested and updating progress on the internal and external systems in line with policy
