Job summary
We are a friendly GP surgery looking for an enthusiastic and experienced Practice Nurse who is looking to progress their career in General Practice. We have a very supportive, flexible, forward thinking nursing and HCA team and pride ourselves on strong team working.
We require someone to work a minimum of 30 hours over 4 days per week but also welcome applicants looking for full time hours.
We offer evening nursing appointments on a Tuesday and Wednesday and therefore the successful applicant will be required to work one night per week until
Full or part time hours to be negotiated.
Main duties of the job
Our ideal candidate will have:
Extensive knowledge and experience in chronic disease management.
Competence in childhood immunisations, cervical cytology screening, contraception management, wound care management and provision of general treatment room care.
A non-medical prescribing qualification would be desirable but not essential.
We are also a training practice and provide placements for Student Nurses in conjunction with The University of Nottingham.

Established Clinical team comprising of two GP Partners, one Advanced Nurse Practitioner Partner, four Salaried GPs, additional GP Registrars/F2s, two Practice Nurses, one trainee Nurse Associate and three Health Care Assistants. This will be in addition to one Senior Practice Pharmacist, one Pharmacy Technician, one First Contact Physiotherapist, one Paramedic and one Mental Health Practitioner as part of the PCN.
List size of 12,000 patients.

We use Ardens template toolkit which provides structure and standardisation for clinical consultations.
Remote consultations offered where appropriate using eConsult and ACCURX.

Job Overview:

To provide nursing services to the practice population by assessment, planning, implementation and evaluation of care
To ensure the provision of a high-quality patient service within a safe, efficient working environment by providing effective support to the Partners and staff and encouraging the development and progress of Highcroft Surgery
Maintaining appropriate records to ensure continuity and quality of care for patients
To collaborate with other GP Partners/employees and practice staff on development and delivery of PBC (Practice Based Commissioning) and enhanced service issues within Highcroft Surgery
To be involved in practice development, identifying service changes and developments in line with clinical needs locally and work to develop these as part of PBC and other processes
Maintaining records of appointments and other administrative duties
To maintain skills and undertake such training and development as may from time-to-time be required to maintain personal competency
Location:
Highcroft Surgery, but you may be asked to travel for meetings and training or provide cover for another practice or Primary Care Network at such notice.

Responsibilities and Duties of the Job
Role Specific Duties:
To provide assessment, treatment, screening, health education services and advice working in accordance with the NMC Code of conduct
Provide nursing treatments to patients independently or in participation with other clinical colleagues working to agreed protocols and guidelines
Provide general and specific health screenings to the practice patients within agreed protocols and guidelines referring to other members of the clinical team as necessary
To manage patients with chronic diseases; running specific clinic sessions and maintaining robust call and recall systems
Participate in national screening work such as cervical cytology and meet targets delivering the national immunisation and vaccination schedule
Nursing procedures to include:
Wound management
Measuring blood pressure
Routine injections as prescribed
Cervical smears
ECG's
Ear syringing
Vaccinations and immunisations
Assisting with minor surgery and IUCD insertion
Provide advice to patients for general health care and minor ailments, with referral to another clinician as necessary
To participate in research within the practice
To be responsible for the efficient running of the treatment room and equipment
To assist with the management of leg ulcers
Select, carry out and interpret results of specific tests
Ensure all statutory regulations ( Infection control, Health & Safety, COSHH, etc) are implemented and adhered to
To maintain accurate, comprehensive records of all consultations and treatments in the patient's notes, both written and computerised
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
Ensure collection of statistical information is required for regular and ad hoc reports
To have working knowledge of equipment used in the surgery
Ensure compliance with patient confidentiality at all times in line with current legislation
Contribute to the development of new patient services
Inform your line manager of any reported or suspected failings in the provision of care within the practice
Working with Others:
Evaluate and adapt care using evidence-based practice in consultation with colleagues as a forum for team education, development and audit
To set up new clinics and monitor existing clinics to ensure smooth running and collection of statistics
To participate in the education of students of all disciplines
To be actively involved and engaged in practice and wider team meetings and forums to promote communication and development
Attend and participate in practice and staff meetings as appropriate
Maintain effective liaison with other agencies and staff concerned with patient care, including other disciplines within the practice, whilst maintaining confidentiality
