    Job summary

    The Haematology department at Stockport NHS Foundation Trust is seeking to recruit an enthusiastic ACP to lead the existing CNS team and provide excellent clinical care. This exciting opportunity involves working as part of an enthusiastic team dedicated to ensuring our Haematology patients are at the forefront of service provision.

    The post holder will provide clinical expertise and advice for haematology patients. They will demonstrate a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of non-malignant and malignant haematological conditions and pre and post chemotherapy patients. Be involved in consultant clinics and assist in pre chemotherapy assessments and planning, and emergency attendance avoidance. They will have a key role within the MDT acting as a liaison between all members of the multi-disciplinary team, to allow for seamless and efficient care.

    They will monitor and improve standards of care through supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will participate in clinical governance within the service by undertaking audit and research as part of the MDT.

    Main duties of the job

    Under the direction of a Consultant Haematologist and following a programme of training and competency assessment the Advanced Practitioner will;

  • Provide advanced levels of clinical practice, knowledge and skills in Haematology
  • Undertake Pre-chemotherapy assessment and physical examination as directed by the Haematology team
  • Assess the need for and order investigations within scope of practice
  • They will have a clear understanding of their boundaries and know when to involve the Consultant Haematologist
  • Manage medications including assessment, review and non-medical prescribing where appropriate and in accordance with legislation.
  • Manage own caseload with decision-making responsibilities fitness for treatment
  • Undertake the process of gaining informed consent
  • Prepare accurate and concise reports both verbally and
  • Some technical and invasive procedures according to their scope of practice g. bone marrow biopsies
  • Facilitating the continuity of care of patients
  • Arranging appropriate pre and post chemotherapy investigations as part of the multidisciplinary team
  • Liaising with Emergency Department for haematology emergencies and arrange admissions if required to maintain coherent service provision

    Job responsibilities

  • Provide a visible clinical profile, having both an advisory and clinical input into patient care as directed by the Lead Clinician.
  • Post treatment care, including assessment, treatment review and identification of treatment induced problems and complications
  • Deploy high level communication skills and effective communication strategies to ensure that information and knowledge is imparted sensitively according to the needs of patients, staff and students in training.
  • A variety of outpatient activities, including seeing patients as and when they are deemed competent to do so
  • Support in-patients as a member of the MDT including attendance at board and ward rounds where required
  • The evaluation of care, including the discharge process and follow-up care arrangements for patients
  • Keep relevant records and submit reports in a professionally acceptable manner. Support patients through the emergency patient pathway from outpatient appointment to discharge following elective admission.
  • Be responsible for the emergency admissions and ensuring patients are being managed effectively along their pathway.
  • To order interpret and act upon medical investigations and expedite access to appropriate medical staff
  • Independently adjust treatment plans (pharmacological and non-pharmacological) according to need assessing appropriate management options and initiate treatment or referral to an appropriate professional where necessary
  • To demonstrate advanced critical thinking and analytical skills to support clinical judgement and decision making process
  • Communicate effectively with a wide range of people in a manner consistent with their level of understanding, culture and background
  • Working in a collaborative manner with all other members of the multidisciplinary team, fostering an environment that is supportive of professional respect and equality.
  • Contributing to the identification of training and development needs within the business group delivering multi-professional education and training as required.
  • To obtain regular clinical supervision and feedback from Consultant mentor, maintain a portfolio of evidence that reflects the maintenance and on-going development of clinical skills.
  • Regularly audit own clinical practice identifying areas for training and development.
  • Identify own development needs, take responsibility for own personal development and maintain a PDP, presented at annual appraisal.
