The Intensive Support Team has an exciting opportunity (due to staff promotion and development) for a skilled, passionate, and dynamic Learning Disability or Mental Health Nurse to work creatively to make a real difference to the lives of Adults with a Learning Disability.

We are looking for individuals who share our strong value-based approach and have an understanding of the support needs of people with a Learning Disability and Autism, who are at risk of an acute hospital admission or placement breakdown.

This post is for a Band 6 Lead Nurse, working 37.5 hours per week on a permanent contract. We are extremely flexible and if you would like to discuss the contracted hours please contact us.

The team works 8am-8pm, Monday to Friday and 9am-5pm, Saturday-Sunday on a rota basis.

This rota also includes an on-call provision which covers 24 hours a day, which is telephone advice only after 8pm. ( On average Lead Nurses within the team will cover x3 on call nights per month)

Our ethos is to work intensively and creatively as an MDT to make a real difference to the lives of Adults with a Learning Disability and Autism.

This is an exciting opportunity to be part of a specialist team and to work closely in collaboration with the wider Joined Up Care Derbyshire organisations.

Main duties of the job

The IST is an innovative, dynamic and responsive team providing support to people with learning disabilities within Derby City and South County.

We work intensely for short periods of time with people to support them at times of crisis and prevent placement breakdown or hospital admission.

We promote person centred practice and work creatively to improve people's quality of life.

The work is mainly community based with some in-reach work into hospitals when required.

The team is working flexibly, and this includes being based in the office and also home working.

To achieve the best results for the people we support, we carry a small caseload compared to community teams. This enables us to work intensively with service users and their families/carers and make a real difference in a short period of time before supporting people onto mainstream or community services.

The post offers an opportunity for you to develop your skills with training, supervision, and an ethos promoting innovative practice; with a focus on 'real life change' for our service users.

We work as a fully integrated multidisciplinary team and take a 'whole team approach' to assessment, formulation and treatment.

Person Specification Qualifications

• RNLD/ RMH Qualification
• Mentorship Qualification/ Standards for Student Supervision & Assessment
• Portfolio of specialist courses appropriate to post
• Educated to degree level

Experience

• Experience of working with service users with an LD/ASD
• Experience of working with service users with mental health needs/ behaviours that challenge
• Experience of working within the Transforming Care Agenda
• Experience and Knowledge of completing Positive Behaviour Support Plans
• Experience of developing and leading staff
• Experience of working in crisis situations
