    Our community nursing teams are closely aligned with local Primary Care Networks, to make sure patient care is holistic and joined up. We believe in treating the whole person, not just the condition, so we take time to get to know our patients.

    Main duties of the job

    Could you use your skills and experience to support the NHS to deliver great patient care? Great patient care needs great people to support clinical teams. That's where you come in.

    Qualifications

    Essential criteria

  • Level 3 qualification (A-Level, City & Guilds Level 3/ NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) and/or equivalent experience (2) Functional Skills at Level 2 (Literacy and numeracy/GCSE grade A-C or equivalent in English and Maths) (2)
  • Desirable criteria

  • Further related CPD; nutrition, physical activity (2) RSPH level 2 in Understanding Behaviour Change and/or Understanding Health Improvement (2)
  • Experience

    Essential criteria

  • Experience of working on own initiative (1) Work (paid or voluntary) in a community setting (1) 1 years' experience in a health or social care setting (1)
  • Desirable criteria

  • At least two years' experience working in a health improvement setting, including experience within community settings (1) Working in a busy environment (1) Handling a range of public, client, professional enquiries (1)
