An exciting opportunity has arisen for us to recruit community focused and creative Band 7 Community Matrons for our iCares South Team.

The South Team will support people to leave hospital earlier and then deliver intensive interventions to continue the recovery and rehab process when the person is in their own home.


You will work closely with the Virtual Ward, UCR and Home Based Intermediate Care MDTs and other teams as well as the daily oversight of the patients in the community ensuring their holistic needs are met in their home.

You will be supported by Consultants and ACPs as well as the wider Integrated Care Service (iCares) which works across Sandwell Community providing rehabilitation, case management and admission avoidance.

There is the opportunity to bring your leadership and clinical skills to help guide our evolving services.

The key duties for the post holder will be;
To provide leadership and oversight of the team working across community.


To be a lead clinician within the service, co-ordinating, delivering and evaluating the care provided to patients on a daily basis.

To provide highly developed specialist assessment and treatment of patients with planned and unplanned complex presentations within community settings.

To supervise, assess and educate team members including bank or agency staff, support workers and students.

As a trust we fully support flexible working and part time workers, which can be factored into your working pattern.

Within iCares we operate a 7 day service 8am-8pm, we take it in turns to contribute to weekend working and 8pm finishes.
