Tempting Recruitment are working on behalf of our client, a

local authority in the

Northeast of England region, to recruit experienced

Home First Practitioner. If you are looking to increase your earnings for the year,

Contract: 3 months (

possible extension)

Salary:
£15 p/h umbrella

Start Date:
ASAP

Shift Pattern: 36 hours per week /Monday - Friday / 9:00am - 5:00pm
**HYBRID WORKING AVAILABLE

DUTIES

Support operational social care teams with ensuring people return home from hospital in a safe and timely manner, through successful partnership working with Health and Social Care.
Facilitate discharge from local hospitals in a timely manner.
Prevent unnecessary hospital admissions by working across community and hospital.
Implement national guidance on discharge requirements.
Provide care and support to residents in a care home environment

JOB REQUIREMENTS

Previous experience working within a local authority or a similar organisation.
At least 2 years' experience within a Home First Practitioner role or equivalent.
Hold a qualification in either social care, health or housing and ideally already hold a Level 3 qualification in Health and Social Care.
Competent Computer Skills
Microsoft office/ word and outlook.
Previous Healthcare Experience, working with patients/customers.
