    This is an exciting new post introducing Personalised Care roles to the Trust. The role is funded to primarily focus on the set-up and implementation of a Treating Tobacco Dependency (TTD) service for in-patients (with the exception of Maternity and staff Services), ensuring the provision of the highest standards of care, services and patient safety in line with the Trust values.

    The post-holder will effectively lead, motivate, line-manage and supervise a small team of Health Coaches working mostly in in-patients wards and will work closely to, and in collaboration with, clinical colleagues and the pharmacy team. The post-holder will also be responsible for the recruitment and induction of new staff and develop the team through pro-active and robust performance review, appraisal, training, coaching and appropriate delegation.

    As a Team Leader you will have a proven track record of service implementation. You will also be responsible and accountable for the safe, efficient and effective management and deployment of staff ensuring full and adequate cover across the service.

    The Treatment Tobacco Dependency service falls within the Trust's Health Inequalities Programme. You will champion Personalised Care principles and help achieve better health outcomes for our patients and reduce health inequalities. The service will establish links with community services and signpost patients to them as appropriate.

    Main duties of the job The post holder will have a small caseload, provide cover during staff absence and at other times required by the service.

    The patient-facing aspect of the role involves providing advice and information to patients on the impact of smoking on their health and treatment, and the use of behaviour change interventions to motivate patients to consider the use of NRT. Interventions will be tailored to maximise impact, taking into consideration patients' circumstances and preferences.

    The team will be for responsible for updating electronic patient records (EPR) and making recommendations regarding their nicotine replacement treatment (NRT), as well as supporting patients to continue with NRT upon discharge, linking them with community services and Primary Care. The post-holder will be required to complete mandatory smoking cessation training once in post (NCSCT).

    The post holder will also work with colleagues across Trust to ensure the Trust is fully compliant with the NICE guideline on Tobacco.
