Main Duties of the Job

  • Seeking a Care Co-ordinator to join our Ageing Well Team for integrated care
  • Work closely with occupational therapists, physio, and clinical care coordinator
  • Collaborate with GP practices to meet healthcare needs and promote independence

Job Responsibilities

  • Take the lead in providing administrative support to the team
  • Develop booking systems and manage patient appointments
  • Facilitate referrals to specialist services and monitor outcomes
  • Collect and analyze data to ensure efficient service delivery
  • Identify population groups that could benefit from our services
  • Establish partnerships with local agencies and community groups
  • Participate in meetings and support clinical staff as needed

Communication

  • Communicate empathetically and adapt as needed
  • Support MDT processes and maintain effective relationships
  • Act as a point of contact for patients, families, and professionals

Administration

  • Maintain accurate patient records and activity data
  • Ensure timely documentation and dissemination to relevant professionals

Other

  • Contribute to service developments and improvements
  • Participate in audits, policy implementation, and training
  • Promote health and safety practices
  • Engage in personal development and training initiatives
