An exciting opportunity has arisen for a HCSW to assist in the service delivery and re-launch of the Discharge Lounge.

The Discharge Lounge operates Monday-Friday 8am-8pm based at both Bournemouth ad Poole sites.

Our aim is to provide a comfortable and safe environment for our patients transferring from hospital to home and to support timely flow and bed availability in UHD.

Applicants will be expected to be flexible to a developing service, have an innovative approach to meet the needs of the patients and service.

Applicants should meet the essential requirements and recent experience working as a HCSW and experience in discharge planning would be desirable.

You should be able to demonstrate excellent communication and organisational skills, to support and enable the nursing and wider multidisciplinary team to provide excellent standards of safe, effective and compassionate care
