The base for this post could be Clay Cross Hospital or The Hub at South Normanton.

working hours per week also considered.


You will have operational management responsibility for a large team of Therapists and Community Nursing team and be required to travel between sites across the region to meet both patients on clinical visits and staff.

You will line manage the development of the team across the service whilst ensuring day to day activities are delivered safely.


You will need excellent communication skills to support you in this role and lead the team delivering excellent community care.

Working as a valued member alongside other senior clinical, specialists and operational leaders covering the South Normanton, Clay Cross, North Wingfield and Tibshelf areas you will be supported by the Integrated Community Manager as well as your colleagues.

You must have professional body registration and be educated to degree level or have an equivalent clinical qualification. You must have clinical NHS experience and developed knowledge of community based healthcare. Operational management experience at a ward or team level is also essential.

To provide clinical and operational leadership to the Integrated Community Teams.

To establish and maintain the clinical culture, effectiveness and performance of the Integrated Community Teams.

To provide full line management responsibilities/duties to designated members of the Integrated Community Teams.


To coordinate the delivery of the collective Integrated Health Care response, working with appropriate partners to ensure services are delivered in the most efficient and responsive manner.

To have high level responsibility for the collective caseload of the Integrated Community Teams.


To implement service and improvement developments within the Integrated Community Teams and work with Integrated Community Managers, (ICM), and peers DCHS wide to influence service standards and provision which impact across ICBS and DCHS services.


Effective Management of designated budget within the Standing Financial Instructions and act as an authorise signatory for ordering equipment, mileage, etc.
