About the Role:

Colchester Medical Practice is seeking a PCN Care Coordinator to join their team

The role involves working closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.

The Care Co-Ordinator's role will support the clinical director and management team in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred.

The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patient's journey through primary care.


Colchester Medical Practice is a full merged large practice with 3 GP surgeries and a new health & wellbeing hub where this role will be based alongside our other care coordinator.

The new hub also accommodates our health & wellbeing coaches, PCN cancer care nurse, child counsellor and adult mental health nurse along with other 3rd party services who we make our rooms available for.

Key duties and responsibilities

The Care Co-ordinator's responsibilities include but are not limited to the following:

To work with the GPs and other primary care professionals within the PCN to identify and help manage caseloads of patients.


To work closely and in partnership with the Social Prescribing Link Workers and Health and Wellbeing Coach(es) where appropriate and available.


To support patients to utilise decision aids, help create single personalised care and support plans, in line with the best practice.

To support the Clinical Director and management team in the delivery of the PCN DES specifications.

To help staff, clinicians and patients to manage their needs through answering queries, making, and managing clinics and appointments.


To provide co-ordination and navigation with the aid of digital tools for people and their carers across health and care services as services become more integrated.

To support the co-ordination and delivery of MDTs such as palliative care meetings within the PCN.

To support and help organise health campaigns and specialist or dedicated clinics and projects.

To promote increased take up to screening services such as cervical screening.

Support QOF Quality Improvement work where required

Supporting and helping to organise and run mass vaccination clinics and also assist in achieving core immunisation performance.

Organising primary care and other linked services for housebound patients.

Manage patient 'did not attend' markers and actions across the PCN

Assisting in keeping referral documents up to date to ensure smooth pathway for referrals.

Provision of care navigation and key skills training for reception staff.


Build and maintain knowledge of all aspects of care linked to primary care in order to act as a central PCN source of assistance to staff and clinicians.


The above duties listed are not exhaustive and being a new role will likely evolve along with the needs of our patients and the practice.

Experience of working in primary care would be an advantage but not essential as full training will be provided.

  • Ideally you will have experience of working within healthcare or the voluntary / community sector, supporting vulnerable groups;
  • Ideally you will have a Diploma Level 2 in Health and Social Care or equivalent qualification but not essential. Good general education essential.
  • You will have strong listening and communication skills;
  • You will be motivated by helping people, with care and empathy;
  • In delivering personcentred support in a nonjudgmental way;
  • Good basis IT skills
  • Excellent time management and organisational skills.
  • Ability to support with complex projects, but also competent and accurate when completing simple repetitive tasks.

Colchester Medical Practice is a unique PCN in north East Essex being a fully merged single practice made up from 4 smaller practices.

We operate on the same clinical computer system and all sites work to the same systems and processes. For a care coordinator this provide a distinct advantage as there are no barriers between independent practices or surgeries. We are already a fully integrated team it will be easy to work with.
