    Job summary

    We are looking to recruit a Clinical Nurse Specialist (CNS) to work within the Leeds Teaching Hospitals Trust (LTHT) Adult Palliative Care Team.

    The Palliative Care Team provides adult specialist palliative care services across all hospitals within LTHT. The CNS role includes working closely with referring medical teams to meet the specialist palliative care needs of patients and families. You will be responsible for managing a caseload of patients within LTHT. The role is primarily one of providing clinical advice, support and education. The post holder will be expected to work weekends and bank holidays.

    Main duties of the job

    We are looking to recruit an enthusiastic individual to join our dynamic team. You should have excellent communication and clinical skills, demonstrating both compassion and the ability to work collaboratively in this challenging but rewarding role. A solid clinical knowledge and the ability to support clinically complex patients is expected.

    You will be supported to develop into the role through a robust induction programme and we actively promote lifelong learning and staff wellbeing.

    There is potential, in the future, for rotation within the team to deliver project work.

    The role involves meeting patients and families on wards, at the invitation of their treating medical team. The patient group we support have life-limiting illness, highly complex needs and are often in the last days or weeks of life. You will be assessing their needs from a specialist palliative care perspective, helping them identify and address their holistic concerns as well as ensuring symptoms are identified, assessed and a management plan suggested.

    Following a supported induction period you will work autonomously, but always with the support of your palliative care team colleagues.

    Education delivery, project work and role modelling / leadership behaviours commensurate with a Band 6 CNS are expected aspects of the role.

    Leeds Teaching Hospitals NHS Trust is a large organisation with more than 2000 in-patient beds on over 70 different ward areas.

    The Palliative Care Team is part of the Oncology CSU.

    The team supports patients at Leeds General Infirmary, St James University Hospital and Chapel Allerton Hospital.

    You will be a part of the multidisciplinary Palliative Care Team. We are an experienced team of doctors, nurses and pharmacist providing care to this complex patient group.

    Job responsibilities

    The post holder is responsible for the management of a defined caseload of patients with specialist palliative care needs. The post holder will carry out the assessment of those needs, working with the treating teams in the planning, implementation and evaluation of measures to support both the patient and those close to them.

    The post holder will also contribute to the development of the palliative care service in LTHT and wider, using resources cost effectively.

    Current services are to provide specialist palliative care, advice and support to patients, families and clinical teams within Leeds Teaching Hospitals.

    Clinical

    Clinical care delivery

    Provide a clinical lead and role model to nursing staff by delivering high quality care.

    Undertakes clinical procedures where dexterity and accuracy are required.

    Assesses, plans, implements and evaluates care and treatments using specialist clinical knowledge and skills.

    Communication

    Maintains accurate clinical and other records using the Leeds Teaching Hospitals Trusts documentation.

    Liaise with Community Services (as appropriate) to ensure seamless provision of care delivery

    Education and Training

    Provide training and education of learners and other junior staff

    Actively participate in the development of own personal development plan and performance review

    Leadership and management

    Investigate and respond to accidents, complaints, untoward incidents and other significant events

    Work with Matron to promote the development of nurses undertaking the CNO Ten Key Roles as considered appropriate

    Under the direction of Matron participates in or leads the development of specific elements of nursing practice, reflecting the CMT and Nursing Directorate business plans

    Participates in the setting, monitoring and improvement of standards and the quality of patient care, including those standards defined in the National Benchmarks for Essential Care

    Promote evidence based practice, wherever possible

    Engages users and carers views on service delivery and development

    Implement policy and practice guidelines specific to the clinical specialty, proposing changes where appropriate

    Contribute to policy and protocol development for speciality area

    Exercises a personal duty of care in relation to equipment, resource and stock control utilising equipment and supplies appropriately.

    Research and Audit

    Contribute to the research agenda

    Undertake audit programmes specific to client group, treatments and developments.

    Participate in clinical trials as appropriate.
