Role Description
- To support young parents and families during pregnancy and/or with children 0 2 to access opportunities and support services that promote community inclusion and connectedness, enabling greater levels of independence through a person centred and responsive approach to individual need.
Main duties and responsibilities
In partnership with SinglePoint, Sandwell Organisations and Family Hubs:
- To provide one-to-one support for parents/carers during pregnancy and/or with children 0-2 years.
- To provide information and practical support, acting as a signpost/navigator to local services for parents/carers, where appropriate.
- To actively build connections with organisations across Sandwell who offer support to families within the project ie. Family Hubs, Early Years Settings, GP's
- To identify, connect and engage with community based parents/carers support groups, providing information and practical support to families who may benefit.
- To identify barriers to access and local need, working creatively to reduce barriers and increase access to local services.
- To offer practical support to parents in the completion of relevant documentation ie
- To provide information about/and promote family enrichment opportunities offered through SinglePoint projects.
- To promote & increase use of the DadPad by fathers & father figures.
- To possess a willingness to support families in a solution focused approach.
- To meet deadlines and manage workload effectively, recording interactions & engagement of families supported.
- To support the completion of family hub research & parent surveys, ensuring that families are regularly consulted about their needs and expectations.
- Implement safeguarding policies and refer issues to SinglePoint Designated Safeguarding Leads.
- To attend Sandwell Family Hubs meetings as required to ensure the voices of parents are heard.
- To regularly update Project Leader of progress and developments, making suggestions for ways to further engage with families by keeping updated on local services and activities.
- Actively promote engagement with partner organisations and their services.
- Engage, encourage, support and motivate families to participate in SinglePoint family activities
- Maintain accurate records and provide information and reports on progress to SinglePoint.
- Use project monitoring tools & other technology, as required.
- Ensure appropriate sensitive handling of confidential information compliant to GDPR.
- Undertake training and professional development as required.
- Assist in the marketing and promotion of SinglePoint.
- To undertake duties in accordance with policies & procedures outlined in the SinglePoint Employee Handbook.
- Participate in and contribute to SinglePoint professional development programmes
- To undertake any other reasonable duties as directed by SinglePoint.
Special requirements
- Ability to travel between multiple community delivery sites across Sandwell
- Enhanced DBS Clearance
