To implement the statutory duties of the Local Authority under the Children and Families Act and procedures relating to the statutory assessment Education, Health and Care Plans (EHCP) for children and young people with special educational needs and/or disability as specified in the Special Educational Needs and Disability (SEND) Code of Practice (2015)


Deliver specialist information, advice and guidance to young people and their parents and carers in a range of settings and in line with the SEND Code of Practice Support young people to achieve the national 'Preparing for Adulthood' outcomes.

Collate all papers and advice relating to EHCPs statutory assessment of children identified with special educational needs, amend and issue EHCPs as required by the Children and Families Act 2014, ensuring confidentiality at all times

Key Responsibilities

  • Maintain a caseload of identified young people and provide a professional, effective, responsive and supportive service to parents of children and young people who are undergoing an EHC statutory assessment, in order that they fully understand their rights, are empowered to express their views regarding the education of their children and signposting parents/carers to the Local Offer and the Special Educational Needs Information and Advice Support Service (SENDIASS)
  • Manage and support the timely planning of transfers to the next phase for all children with statements, to ensure that all change of phase statements are consulted on and amended by 15th February in the year of transfer. To provide accurate information and appropriate advice to parents/carers, schools and other agencies regarding the change of phase review process, statutory timescales, sources of independent advice and the range of provision available.
  • Deliver specialist careers information, advice and guidance to young people and their parents and carers in a range of settings and in line with the SEND Code of Practice 2015 and contributing to multiagency assessments in order to facilitate young people's successful transition into appropriate Preparing for Adulthood outcomes.
  • Notify and monitor the completion of annual reviews by all settings and have in place a systematic process to chase outstanding reviews including direct contact with Special Educational Needs and Disability Coordinators (SENDCOs in schools and settings.
  • Act as a the key link with local and outofarea educational institutions, training providers and related agencies to ensure the quality and appropriateness of the support, training, educational and employment opportunities offered to young people with additional support needs.
  • Be responsible for the input of data on Capita for pupil progression and work with Senior SEN Officers to establish that all pupils with EHCPs are making good progress towards the outcomes in their EHCP.
  • Work with colleagues and partner organisations to follow up and support vulnerable young people are Not Education Employment or Training (NEET) or who are at risk of becoming disengaged from education, employment and training.
  • Act as a the key link with local and outofarea educational institutions, training providers and related agencies to ensure the quality and appropriateness of the support, training, educational and employment opportunities offered to young people with additional support needs.
  • Amend Education, Health and Care Plans in an accurate and concise manner within the specified statutory timescales based on the written advice provided

Knowledge:

  • Excellent knowledge understanding and experience of the Children and Families Act 2014, Code of Practice 201
