    Job Summary

    To support the practice team by signposting patients to the appropriate healthcare professional or service, working as part of the practices multidisciplinary team.

    The care navigator will also be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.

    Job responsibilities

    Job Summary

    Tosupport the practice team by signposting patients to the appropriate healthcareprofessional or service, working as part of the practices multidisciplinaryteam.

    The care navigator will also be expected to undertake receptionduties as part of the role, supporting the administrative team in delivering apolite and professional service to the entitled patient population.

    Primary Key responsibilities:

    Thefollowing are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement tocarry out other tasks. This will be dependent upon factors such as workload andstaffing levels and training:

    Processand effectively signpost patients to the appropriate healthcare professional,depending on the presenting condition

    Maintainand monitor the practice appointment system

    Answerincoming phone calls, transferring calls or dealing with the callers requestappropriately

    Maintainand Monitor all online requests/Econsults are action in a timely manner

    Initiatingcontact with and responding to, requests from patients, team members andexternal agencies

    Clinicallycode data on TPP System 1 accurately

    Photocopydocumentation as required

    Dataentry of new and temporary registrations and relevant patient information asrequired

    Ensureall queries for online registrations are auctioning promptly

    Inputdata into patients healthcare records as necessary

    Directrequests for information, , SAR, insurance/solicitors letters and DVLAforms to the administrative team

    Manageall queries as necessary in an efficient manner

    Carryout system searches as requested

    Maintaina clean, tidy, effective working area at all times

    Monitorand maintain the reception area and notice boards

    Supportall Practice staff with general tasks as requested

    Participatein audit as directed by the audit lead

    SupportCare Navigators/Administrators, providing cover during staff absences

    Actionincoming emails and correspondence as necessary

    Scanpatient related documentation and attach scanned documents to patientshealthcare records and code accurately.

    Completeopening and closing procedures in accordance with the duty rota

    Asrequired support POD Team in the management of repeat prescriptions, ensuringthey are processed accurately and efficiently.

    Produce,maintain and participate in the Care Navigators rota for lunch and out of hourscover

    Undertakeall mandatory training and induction programmes, and ensuring all mandatorytraining is maintained and up to date

    Supportthe with the Patient Participation Group related matters

    Supportthe health promotion lead and display promotional material on the allocatednoticed boards and in the waiting room

    To liaise and arrange meetings as required and to attend meetings andtake minutes

    To retrieve medical records and assist the completion ofmedical/insurance records

    Attenda formal appraisal with their manager at least every 12 months. Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed

    Contributeto public health campaigns (, flu clinics) through advice or direct care
