    Job summary

    An exciting opportunity has arisen within the Trust wide tissue viability team to cover maternity leave. The post holder will be a dynamic individual who willmotivate health professionals to promote and develop nursing care in all areasof tissue viability; creating and maintaining a culture of effective patientand health care services. To promote pressure ulcer prevention and reduce patient harm.

    The post is multifaceted as it has clinical, educational,audit, research and development components.The post holder will have skills and knowledge in the field of tissueviability, you will be required to work autonomously in performing assessments of wounds and implement wound management andpressure ulcer prevention plans. You will provide advice and support tohealth care professionals within the Trust and across Mid Yorkshire Communityto improve care delivery and outcomes with support from senior Tissue Viabilitycolleagues.

    Please note that there are 2 positions available; 1 x 30 hours per week and 1 x 24 hours per week.

    Main duties of the job

    The post holder will provide advice and intervention to all patient groups withtissue viability needs, promote a seamless service within a multi-professionalframework and will contribute to the development of the tissue viabilityservice. You will be responsible for theprovision of advice and support to Trust and non-Trust healthcareprofessionals in relation to Tissue Viability. Provide education and training onTissue Viability, including wound management, pressure ulcer prevention,pressure relieving equipment and leg ulcer management.

    To discuss/demonstrate dressingchoices/techniques for wounds at request of referring professional.

    Liaise with acute Trust,community district nurses, residential homes, nursing homes, and otherdisciplines regarding the management, discharge and transfer of patients with Tissue Viability requirements.

    The post holder will contributeto achieving the objectives of the Trust by implementing quality improvementstrategies in relation to pressure ulcer prevention to reduce trust acquiredpatient harm.

    Take an active part in thedevelopment of nursing in the Trust via participation in professional nursingforum and shared governance initiatives.

    Job responsibilities

  • To provide clinical, social andpsychological support to their patients using excellent communication skills,education and health promotion to enable them to reach their optimum level ofindependence and health
  • To provide advice, supervisionand support to their defined caseload and peers.
  • Responsible for providing evidence basedpractice
  • Will have responsibility for supporting the Matronfor Tissue Viability in leading identified and agreed aspects of the service,in the development of care systems, protocols and guidelines working as part ofa multi-disciplinary team and developing excellent interdepartmental relationships with excellent communicationwith a focus on quality improvement
  • Work collaboratively with the TissueViability Team and Allied Health Professionals, with in their own scope ofpractice
  • Will ensure cost effective use of allresources
  • Promote programmes of health education andhealth promotion relevant to service
  • Work within national policy frameworksrelevant to the service
  • Deal effectively and professionally withpatient / service user issues collaborating with PALS as appropriate and take aproactive approach to resolving issues and learning from complaints andescalate when unable to resolve
  • Contribute to the multi-professionalapproach in the management of patients /carers within the specialist area in relation to ongoing care needs
  • Act as a resource for tissue viability knowledgefor clinical staff in the Care Closer to Home division.

  • Qualifications

    Essential

  • Registered General Nurse Adult part 1.
  • First level degree or demonstrable experience in Tissue Viability.
  • Evidence of continued professional development.
  • Desirable

  • Recognised teaching qualification.
  • Non-medical prescribing.
  • Completion of Tissue Viability / wound care course.
  • Recognised qualification in sharp debridement.
