The aim of this service is to provide support for inpatients and utilising their PHB to facilitate personalised care back into the community, this can be done by providing goods or a service to ease their transition of care from inpatient to community services.


The role will appeal to those who have:

  • Have experience of working with adults in a health, social care or any other relevant setting
  • Have a good knowledge of services across the Bradford District & Craven commitment to excellence
  • A passion to help improve people's lives
  • Have excellent communication skills and feel confident in supporting people in a clear, meaningful and nonjudgemental way
  • A commitment to excellence
  • Have good organisational, financial and IT skills
  • Are confident in recognising and responding to risk and safeguarding concerns
  • Ability to take initiative and be proactive in solutions
  • Passion for multiagency working
  • Willingness to travel across the District (Full UK driving licence & have access to a car)

Important information

Salary:
£12 - £13.14 per hour

Contract:
Temporary for 12 months with the potential for extension dependent on funding

Hours: 20 hours per week

Probation: 6 months

Wednesday 29th & Thursday 30th May 2024


Applicants to be able to work evenings and support the on-call rota as and when required
